毫针速刺法结合推拿对脑卒中软瘫期肩痛疗效及病灶超声检测指标的影响 被引量:4

摘要 目的通过观察毫针速刺法结合推拿对脑卒中软瘫期肩痛疗效及病灶超声检测指标的影响,以便为针灸治疗此病提供高效、安全的方法。方法将102例脑卒中软瘫期肩痛患者以随机数字表法分研究组(51例)、对照组(51例)。对照组给予常规康复训练联合推拿治疗,研究组在对照组的基础上另给予毫针速刺法,两组均连续治疗4周后评估疗效。对比两组临床疗效,比较两组治疗前后肩痛、肩关节活动度、肩关节功能及肌骨彩超情况。记录两组治疗安全性。结果研究组总有效率高于对照组(P<0.05)。两组治疗后疼痛视觉模拟评分(VAS)均低于治疗前(P<0.05),且研究组治疗后VAS评分低于对照组(P<0.05)。两组治疗后屈曲、外展、外旋水平均高于治疗前(P<0.05),且研究组治疗后屈曲、外展、外旋水平均高于对照组(P<0.05)。两组治疗后美国加州大学肩关节评分(UCLA)均高于治疗前(P<0.05),且研究组治疗后UCLA评分高于对照组(P<0.05)。两组治疗后盂肱关节下方关节囊厚度、肩峰至大结节间距均低于治疗前(P<0.05),且研究组治疗后盂肱关节下方关节囊厚度、肩峰至大结节间距均低于对照组(P<0.05)。毫针速刺法结合推拿治疗脑卒中软瘫期肩痛未发生任何不良事件。结论毫针速刺法结合推拿可明显降低脑卒中软瘫期肩痛患者肩关节疼痛程度,改善肩关节活动度、肩关节功能及肩周组织解剖学异常结构,临床疗效佳,且安全性高,值得临床借鉴。 Objective To observe the effect of the fast filiform needling combined with Tuina on the curative effect of shoulder pain in the soft paralysis stage of stroke and the influence on the ultrasonic detection index of the lesion,so as to provide an efficient and safe method for acupuncture and moxibustion treatment of this disease.Methods A total of 102 patients with shoulder painin the soft paralysis stage of stroke were divided into the study group(51 cases)and the control group(51 cases)by a random number table.The control group was given conventional rehabilitation training combined with Tuina therapy,and the study group was given fast filiform needling on the basis of the control group.Both groups were given continuous treatment for 4 weeks to evaluate the curative effect.The clinical effects of the two groups were compared,and the shoulder pain,the motion range of the shoulder joint,the shoulder joint function and musculoskeletal color ultrasound were compared between the two groups before and after treatment.The treatment safety of the two groups was recorded and compared.Results The total effective rate of the study group was higher than that of the control group(P<0.05).The visual analogue score(VAS)after treatment in the two groups was lower than that before treatment(P<0.05)and the VAS score of the study group after treatment was lower than that of the control group(P<0.05).The levels of flexion,abduction and external rotation after treatment in the two groups were high-er than those before treatment(P<0.05),and those in the study group after treatment were higher than those of the control group(P<0.05).The UCLA score of the University of California after treatment in both groups was higher than that before treatment(P<0.05),and the UCLA score of the study group after treatment was higher than that of the control group(P<0.05).The thickness of the joint capsule below the glenohumeral joint and the distance between the acromion and the greater tubercle after treatment in the two groups were lower than those before treatment(P<0.05),and the thickness of the joint capsule below the glenohumeral joint and the distance between the acromion and the greater tubercle in the study group were all lower than those of the control group(P<0.05).There were no adverse events in the treatment of shoulder pain in the soft paralysis stage of stroke.Conclusion The fast filiform needling combined with Tuina can significantly relieve the pain of patients with shoulder pain in the soft paralysis stage of stroke,and improve the mobility and function of the shoulder joint and relieve the abnormal structure of shoulder tissues.It has good clinical efficacy and high safety.It is worthy of clinical reference.
