摘要
目的探讨床旁肺部超声可视化联合CBL教学模式在急诊科住培医师中的教学效果。方法选择2021年1~6月在广西医科大学第二附属医院急诊科轮转的住培医师为研究对象,随机分为观察组和对照组,均进行常规气胸小讲课教学活动。在教学查房中,对照组采用传统幻灯片小讲课方法教学,观察组则采用床旁肺部超声可视化联合CBL教学模式进行教学。观察比较两组住培医师在小讲课后理论考核成绩,并采用自我评价表测定两组住培医师实践能力、应急能力、解决问题能力以及临床思维能力4项内容,采用调查表进行多维度的课程评价教学效果。结果两组住培医师在小讲课后理论考核成绩比较,差异有统计学意义(P<0.05)。在自我评价表测定中,两组住培医师实践能力、应急能力、解决问题能力及临床思维能力得分比较,差异均有统计学意义(均P<0.05)。在调查表多维度的课程评价中,住培医师及急诊住培教学督导组对两种教学模式评分比较,差异均有统计学意义(均P<0.05)。结论床旁肺部超声可视化联合CBL教学模式对提高住培医师快速识别气胸及诊疗、培养临床思维能力方面有显著意义,值得在教学中推广。
Objective To explore the application of bedside lung ultrasound visualization combined with CBL teaching model of the teaching in emergency department resident standardized training.Methods Residents rotating in the emergency department of the Second Affiliated Hospital of Guangxi Medical University from Jan 2021 to June 2021 were randomly divided into an observation group and a control group,and pneumothorax mini-lecture teaching activities were routinely performed.In the teaching visit,the control group used the traditional slide mini-lecture and the observation group used bedside lung ultrasound imaging combined with CBL teaching model.The theoretical assessment scores after the mini-lecture were collected from both groups;a self-evaluation form was used to evaluate the four elements of practical ability,emergency response ability,problem-solving ability and clinical thinking ability of both groups with a score of 25,the higher the value of each score,the more significant the teaching method;a questionnaire was used to conduct a multidimensional course evaluation,which was jointly evaluated by the residents and the emergency medicine resident teaching supervision group,with a score as the higher the score,the more significant the teaching method.Results There was a statistically significant difference in the post-checkout test scores of the residents between the two groups(P<0.05).In the self-evaluation scale measure,there was a statistically significant difference(P<0.05 for each)in the scores of practical skills,emergency response skills,problem-solving skills and clinical thinking skills between the two groups of residents.In the multidimensional course evaluation of the questionnaire,there was a statistically significant difference between the scores of the residents in the two teaching modalities and the teaching supervision group of the acute care residents(P<0.05).Conclusion Bedside lung ultrasound visualization combined with CBL teaching model is important to improve the rapid recognition and treatment of pneumothorax and develop residents'clinical thinking skills,which is worth promoting in teaching.
