无创呼吸机治疗重症小儿肺炎并发急性呼吸衰竭的临床效果分析 被引量:9

Analysis of Clinical Effect of Non-Invasive Ventilator on Acute Respiratory Failure in Severe Pediatric Pneumonia

摘要 目的:探讨无创呼吸机治疗重症小儿肺炎并发急性呼吸衰竭的临床效果。方法:随机选择2019年4月~2020年4月本院收治的重症肺炎并发急性呼吸衰竭的患儿92例作为研究对象,以数字法将患儿进行分组,其中有46例患儿为对照组,为患儿实施鼻导管吸氧治疗,另外观察组46例采用无创呼吸机治疗,对比两组患儿的治疗效果。结果:对照组患儿以及观察组患儿的总体有效率分别为71.4%和95.65%,观察组优于对照组,P<0.05。此外,治疗前,两组患儿的PaO_(2)、PaCO_(2)、SpO_(2)、pH值相比之下,差异不存在统计学意义,P>0.05,治疗后观察组患儿的pH值(7.36±0.26)、PaO_(2)(79.61±4.49)mmHg、SpO_(2)(0.94±0.12)kPa、PaCO_(2)(52.37±4.25)mmHg,对照组患儿的pH值(6.53±0.28)、PaO_(2)(68.55±3.23)mmHg、SpO_(2)(0.72±0.09)kPa、PaCO_(2)(61.27±.91)mmHg,两组患儿的血气指标相比,观察组患儿更加优越,P<0.05;经过治疗后,观察组患儿的FEV_(1)(5.33±0.37)L、FVC(30.79±4.81)L、FEV_(1)/FVC(12.49±3.14)%,对照组患儿的FEV_(1)(3.84±0.26)L、FVC(23.57±2.06)L、FEV_(1)/FVC(9.24±2.33)%,观察组患儿相比对照组患儿的肺功能指标明显更优,P<0.05;而两组患儿在治疗前的生活质量评分(61.97±2.49)分、(61.22±2.61)分,相比差异无统计学意义,P>0.05,治疗后观察组患儿的生活质量评分(87.48±2.94)分,相比于对照组患儿的生活质量评分(83.66±2.75)分更高,P<0.05,两组患儿的治疗前与治疗后生活质量存在差异,P<0.05。结论:针对重症肺炎并发急性呼吸衰竭的患儿,临床上运用无创呼吸机治疗,效果更理想,能够有效改善患儿的呼吸功能及肺功能,有利于患儿的早日康复。 Objective:To explore the clinical effect of non-invasive ventilator in the treatment of severe pediatric pneumonia complicated with acute respiratory failure.Methods:A total of 92 children with severe pneumonia complicated by acute respiratory failure who were admitted to our hospital from April 2019 to April 2020 were randomly selected as the research objects,and the children were grouped by numerical method,of which 46 children were the control group.In addition,46 patients in the observation group were treated with non-invasive ventilator,and the therapeutic effects of the two groups were compared.Results:The overall effective rates of children in the control group and observation group were 71.4%and 95.65%,respectively,and the observation group was better than the control group,P<0.05.In addition,before treatment,there was no significant difference in PaO_(2),PaCO_(2),SpO_(2)and pH values between the two groups,P>0.05.After treatment,the pH values of the observation group were(7.36±0.26),PaO_(2)(79.61±4.49)mmHg,SpO_(2)(0.94±0.12)kPa,PaCO_(2)(52.37±4.25)mmHg,pH value of control group(6.53±0.28),PaO_(2)(68.55±3.23)mmHg,SpO_(2)(0.72±0.09)kPa,PaCO_(2)(61.27±.91)mmHg,compared with the blood gas indexes of the two groups,the observation group was more superior,P<0.05;After treatment,the FEV_(1)(5.33±0.37)L,FVC(30.79±4.81)L,FEV_(1)/FVC(12.49±3.14)%of the observation group,the FEV_(1)(3.84±0.26)L,FVC(23.57±2.06)L,FEV_(1)/FVC(9.24±2.33)%,the pulmonary function index of the observation group was significantly better than that of the control group,P<0.05;The quality of life scores of the two groups before treatment were(61.97±2.49)points and(61.22±2.61)points,and there was no significant difference,P>0.05.After treatment,the quality of life scores of the observation group were(87.48±2.94)points,which was higher than that of the control group(83.66±2.75)points,P<0.05.There was a difference in the quality of life between the two groups before and after treatment,P<0.05.Conclusion:For children with severe pneumonia complicated by acute respiratory failure,the clinical use of non-invasive ventilator therapy is more effective,which can effectively improve the respiratory function and lung function of children,and is conducive to the early recovery of children.
