摘要
目的比较经尿道前列腺电切术(TURP)与经尿道等离子前列腺剜除术(TUERP)治疗大体积良性前列腺增生的效果。方法回顾性分析治疗的127例前列腺增生(BPH)患者临床资料,其中TURP组51例,TUERP组76例,术前前列腺体积均在80 mL以上,比较2组手术时间、失血量、术后冲洗时间、留置尿管时间、术后住院时间、电切综合征发生率、包膜穿孔率、尿道狭窄率、尿失禁发生率、IPSS评分、尿流率(Qmax)以及IIEF-5评分。结果TUERP组失血量、术后冲洗时间、留置尿管时间、术后住院时间均低于TURP组(P<0.05);TUERP组电切综合征发生率、包膜穿孔率、术后尿道狭窄率均低于TURP组(P<0.05)。术后随访3个月,TUERP组IPSS评分为(6.70±1.03)分,低于TURP组的(14.29±1.27)分(P<0.05);TUERP组Qmax为(21.53±2.29)mL/s,高于TURP组的(17.59±2.28)mL/s(P<0.05);术后随访3个月,TUERP组IIEF-5评分为(16.66±2.32)分,高于TURP组的(14.37±2.07)分(P<0.05)。结论与TURP比较,TUERP治疗大体积前列腺增生,腺体切除彻底,术中出血少,并发症发生率低,住院时间短。TUERP在保护患者尿控、性功能方面更有优势。
Objective To compare the efficacy and safety of transurethral resection of prostate(TURP)and bipolar plasma kinetic transurethral resection of the prostate(TUERP)in the treatment of large volume benign prostatic hyperplasia(BPH).Methods The data of 127 patients with BPH were retrospectively analyzed,including 51 patients in TURP group and 76 patients in TUERP group.The preoperative prostate volume was above 80 ml.Results Compared with TUERP and TURP,the operative time of the two groups was(66.20±8.02)min and(66.18±7.99)min respectively(P>0.05).The blood loss was(76.63±12.76)mL and(126.20±16.57)mL(P<0.05);postoperative irrigation time was(0.95±0.31)days and(2.16±0.45)days(P<0.05);time of indwelling catheter was(3.07±3.0)days and(5.47±0.67)days(P<0.05);postoperative hospital stay was(4.07±3.0)days and(7.14±0.53)days(P<0.05).The incidence of TURP syndrome was 0%and 2%,respectively.The perforation rate of prostate capsule was 0%and 3.9%.The rate of postoperative urethral stricture was 1.3%and 3.9%.The rate of urinary incontinence 1 week after surgery was 7.9%and 2%.The incidence of urinary incontinence at 3 months after operation was 1.3%and 2%.IPSS score at 3 months after operation was(6.70±1.03)and(14.29±1.27)(P<0.05).Qmax were(21.53±2.29)and(17.59±2.28)mL/s(P<0.05);IIEF-5 score was(16.66±2.32)and(14.37±2.07)(P<0.05).Conclusion Compared with TURP,TUERP is more effective in the treatment of BPH with more complete gland resection,less intraoperative bleeding,lower complication rate and shorter hospital stay.TUERP has more advantages in protecting patient’s urinary control and sexual function.
