抗青光眼手术前后睑板腺形态变化的临床观察

Clinical observation of meibomian gland morphological changes on before and after anti-glaucoma surgery

摘要 目的通过分析抗青光眼手术前后睑板腺形态的变化,评估该手术与睑板腺功能障碍的关系。方法98例(107眼)行抗青光眼手术治疗患者,分别于术前1周、术后6个月采用OSDI及OCULUS眼表综合分析仪进行眼表评估。比较患者术前1周、术后6个月OSDI评分;患者手术眼、未手术眼术前1周、术后6个月睑板腺缺失程度评分。结果术后6个月,患者OSDI评分(38.84±7.62)分高于术前1周的(26.47±8.18)分,差异有统计学意义(P<0.05)。术后6个月,手术眼睑板腺缺失程度评分(2.94±0.47)分高于术前1周的(2.17±0.53)分,差异有统计学意义(P<0.05);未手术眼睑板腺缺失程度评分与术前1周比较差异无统计学意义(P>0.05)。结论抗青光眼手术术后可加重睑板腺功能障碍,需要重视围手术期眼表的管理以减少术后干眼的发生。 Objective To evaluate the correlation between morphology changes and dysfunction of meibomian gland by analyzing the morphological changes of meibomian gland before and after anti-glaucoma surgery.Methods There were 98 patients(107 eyes)who underwent anti-glaucoma surgery,and the ocular surface was evaluated by OSDI and OCULUS ocular surface integrated analyzer at 1 week and 6 months postoperatively.Comparison was made on OSDI scores at 1 week preoperatively and 6 months after surgery,the degree of meibomian gland deficiency score at 1 week preoperatively and 6 months after surgery in the operated and unoperated eyes.Results At 6 months postoperatively,the OSDI score(38.84±7.62)points was higher than(26.47±8.18)points at 1 week preoperatively,and the difference was statistically significant(P<0.05).At 6 months postoperatively,the meibomian gland deficiency score(2.94±0.47)points was higher than(2.17±0.53)points at 1 week preoperatively,and the difference was statistically significant(P<0.05);there was no statistically significant difference in meibomian gland deficiency score in the unoperated eyes compared with that at 1 week preoperatively(P>0.05).Conclusion Anti-glaucoma surgery can aggravate the dysfunction of meibomian gland,and attention should be paid to the management of the ocular surface during surgery to reduce the occurrence of postoperative dry eye.
