摘要
目的:观察正中神经滑动术联合甲钴胺口服对腕管综合征患者的疼痛、症状和神经传导的影响。方法:选取2016年1月至2019年12月门诊就诊的腕管综合征患者60例,按入院顺序随机数字表法分为观察组和对照组各30例。两组患者均接受药物甲钴胺口服治疗,观察组患者每日辅以正中神经滑动术治疗。于治疗前、治疗后使用视觉模拟评分法(VAS)和整体症状分数表(GSS)分别对患者的疼痛和症状进行评价。结果:治疗后,两组患者的VAS评分较治疗前降低(P<0.05),且观察组治疗后的VAS评分低于对照组(P<0.05)。治疗后,两组患者整体症状分数表(GSS)疼痛、麻木、刺痛、肌力减退、夜间疼痛、总分较治疗前下降(P<0.05),且观察组整体症状分数表(GSS)疼痛、麻木、刺痛、肌力减退、夜间疼痛、总分明显低于对照组(P<0.05)。治疗后,观察组患者正中神经感觉传导(腕-中指)速度、动作电位波幅,正中神经运动传导复合肌肉动作电位较治疗前明显上升,且明显高于对照组(P<0.05);正中神经运动传导末端潜伏期较治疗前明显下降,且明显低于对照组(P<0.05)。结论:正中神经滑动术联合甲钴胺口服对早中期腕管综合征患者的疼痛、症状均有明显改善作用,值得临床推广。
Objective:To observe the effect of median nerve sliding surgery combined with oral mecobalamin on pain,symptoms and nerve conduction in patients with carpal tunnel syndrome.Method:60 patients with carpal tunnel syndrome from January 2016 to December 2019 were randomly divided into observation group and control group.Both groups were treated with Mecobalamin orally,and the patients in the observation group were treated with median nerve sliding every day.The patients’ pain and symptoms were evaluated by visual analogue scale(VAS) and global symptom score(GSS) before and after treatment.Results:After treatment,the VAS score of the two groups was lower than that before treatment(P<0.05),and the VAS score of the observation group was lower than that of the control group(.P<0.05).After treatment,the total scores of GSS pain,numbness,tingling,muscle weakness,nocturnal pain in the two groups were lower than those before treatment(P<0.05),and the total scores of GSS pain,numbness,tingling,muscle weakness,nocturnal pain in the observation group were significantly lower than those in the control group(P<0.05) After treatment,the median nerve sensory conduction(wrist middle finger) velocity,action potential amplitude and median nerve motor conduction compound muscle action potential in the observation group were significantly higher than those before treatment(P<0.05);The latency at the end of median nerve motor conduction was significantly lower than that before treatment,and was significantly lower than that in the control group(P<0.05).Conclusion:Median nerve sliding combined with Mecobalamin can significantly improve the pain and symptoms of patients with early and middle carpal tunnel syndrome,which is worthy of clinical promotion.
