摘要
目的通过Meta分析系统评价等速肌力训练对脑卒中偏瘫患者运动能力恢复的效果。方法计算机检索建库至2021年3月的Pubmed、EMbase、Web of Science、The Cochrane Library、CNKI、万方医学6个数据库,对搜索到的文献进行筛选、数据提取、质量评价等,用软件RevMan5.3进行Meta分析。结果纳入27篇文献,共1706名研究对象,对照组854名,试验组852名,等速肌力训练改善脑卒中偏瘫患者运动功能Fugl-Meyer评分(FMA)[MD=6.73,95%CI为(5.40,8.06),Z=9.92,P<0.01],平衡功能Berg平衡量表(BBS)[MD=9.03,95%CI为(8.20,9.86),Z=21.32,P<0.01],最大步行速度[SMD=1.31,95%CI为(1.09,1.53),Z=11.66,P<0.01],以上三个结局指标试验组均优于对照组,且具有统计学意义。结论等速肌力训练在一定程度上可以改善脑卒中偏瘫患者运动能力,但由于本次研究所纳入的文献数量和质量有限,所以仍需要开展大样本高质量的临床试验对等速肌力训练的康复疗效加以验证。
Objective To systematically evaluate the effect of isokinetic muscle strength training on motor ability recovery of stroke patients with hemiplegia through a meta-analysis.Methods Six databases including PubMed,Embase,Web of Science,The Cochrane Library,CNKI and Wanfang Medical were searched by computer until March 2021.The literatures were screened,extracted and evaluated for quality.Meta-analysis was performed by RevMan5.3.Results 27 articles were included,a total of 1706 research object,854 patients in the control group,852 in experimental group.The isokinetic muscle strength training was used to improve motor function in patients with cerebral apoplexy hemiplegia.The Fugl-Meyer score(FMA)[MD=6.73,95%CI was(5.40,8.06),Z=9.92,P<0.01],balancing function(BBS)the Berg balance scale[MD=9.03,95%CI was(8.20,9.86),Z=21.32,P<0.01],and the maximum walking speed[SMD=1.31,95%CI:(1.09,1.53),Z=11.66,P<0.01]in the experimental group were all superior to the control group,and had statistical significance.Conclusion To a certain extent,isokinetic muscle strength training can improve the motor ability of stroke patients with hemiplegia.However,due to the limited number and quality of literatures included in this study,large sample and high-quality clinical trials are still needed to verify the rehabilitation efficacy of isokinetic muscle strength training.
