摘要
目的:探究皮炎湿疹患者微生物感染情况,并就其与骨桥蛋白(OPN)表达情况的相关性进行分析。方法:选择2019年5月-2021年6月于盘锦辽油宝石花医院接受治疗的100例皮炎湿疹患者为研究组,另选同期于盘锦辽油宝石花医院接受体格检查的50例健康个体为对照组。分别对研究组患者皮损部位、非皮损部位及对照组正常皮肤样本进行细菌、真菌培养,实施菌种鉴定及菌落计数分析,并就两组个体血清OPN表达情况进行对比,分析细菌真菌感染与皮炎湿疹患者OPN表达情况的相关性。结果:对照组皮肤样本金黄色葡萄球菌、表皮葡萄球菌、马拉色菌感染率均低于研究组皮损样本,表皮葡萄球菌低于研究组非皮损样本,差异均有统计学意义(P<0.05)。皮损样本细菌真菌密度、金黄色葡萄球菌密度、OPN水平均高于非皮损样本(P<0.05),急性渗出型皮损细菌真菌密度、金黄色葡萄球菌密度及OPN水平均高于慢性斑块型皮损(P<0.05)。研究组100例患者中,后期发生继发感染18例,未继发感染82例,研究组继发感染和未继发感染患者细菌、金黄色葡萄球菌及马拉色菌检出率均高于对照组(P<0.05),且继发感染患者细菌、金黄色葡萄球菌检出率均高于未继发感染患者(P<0.05)。研究组细菌真菌定植密度与其血清OPN表达水平呈明显正相关(r=0.7491,P=0.0003)。结论:微生物定植与皮炎湿疹的发生和发展关系密切,患者血清OPN水平会明显升高,可以考虑将其应用于皮炎湿疹诊疗鉴别中。
Objective:To explore the microbial infection in patients with dermatitis and eczema,and to analyze its correlation with the expression of osteopontin(OPN).Method:A total of 100 patients with dermatitis and eczema who received treatment in Panjin Liaoyou Baoshihua Hospital from May 2019 to June 2021 were selected as the research group,and 50 healthy individuals who received physical examinations in Panjin Liaoyou Baoshihua Hospital during the same period were selected as the control group.Bacteria and fungi were cultured on skin lesions,non-skin lesions,and normal skin samples of the control group.The identification of bacteria and the analysis of colony count were carried out.The serum OPN expression of two groups were compared,and the correlation between bacterial and fungal infection and OPN expression in patients with dermatitis and eczema was analyzed.Result:The infection rate of Staphylococcus aureus,Staphylococcus epidermidis and Malassezia infection in the skin samples of the control group were lower than those in the skin lesion samples of the study group,Staphylococcus epidermidis was lower than that in the non-lesion samples of the study group,and the differences were statistically significant(P<0.05).The bacterial and fungal density,Staphylococcus aureus density and OPN level of skin lesion samples were higher than those of non-lesion samples(P<0.05).The bacterial and fungal density,Staphylococcus aureus density and OPN level of acute exudative skin lesions were higher than those of chronic plaque-type skin lesions(P<0.05).Among 100 patients in the study group,18 cases developed secondary infections in the later period,82 cases did not develop secondary infections.The detection rates of bacteria,Staphylococcus aureus and Malassezia in the study group were significantly higher than those in the control group(P<0.05),and the detection rates of Staphylococcus aureus and bacteria in patients with secondary infection were significantly higher than those in patients without secondary infection(P<0.05).There was a significant positive correlation between the colonization density of bacteria and fungi and the expression level of serum OPN in the study group(r=0.7491,P=0.0003).Conclusion:Microbial colonization is closely related to the occurrence and development of dermatitis and eczema,the level of serum OPN in patients will be significantly increased,it can be considered for the diagnosis and treatment of dermatitis and eczema.
