摘要
温病理论作为中医理论不可或缺的一部分,卫气营血理论、伏邪理论、三焦辨证是温病理论的重要组成部分,提供了一个有别于单纯的瘀血痰浊或脏腑功能失调而致病的理论,从一个动态的、更加全面的角度理解探讨银屑病性关节发病机制及其辨治。银屑病性关节炎主要表现有皮损和关节症状,根据其皮损的性质可以从卫气营血辨证以了解病位指导治疗。其病机为邪伏于里,治疗上以透邪为要。其关节症状当属温病湿热痹范畴,治疗应以“祛湿”为要。从三焦辨证治疗痹证时,上焦应注重开肺气,宣畅气机,中焦当健脾以祛湿,下焦应兼补益肝肾,以强筋壮骨。现从这些角度探索其在银屑病关节炎的皮损和关节炎中的应用,从中医角度来认识银屑病性关节炎,开拓临床诊疗思路。
The theory of febrile disease is an indispensable part of the theory of traditional Chinese medicine.Weiqi Yingxue theory,Fu Xie theory and Sanjiao syndrome differentiation theory are important parts of febrile disease theory,which provides a theory different from simple blood stasis and phlegm turbidity or viscera dysfunction.From a dynamic and more comprehensive point of view to explore the pathogenesis of psoriatic joint and its diagnosis and treatment.Psoriatic arthritis is mainly characterized by skin lesions and joint symptoms,according to the nature of its lesions can be dialectical from Weiqi Yingxue to understand the location of the disease to guide treatment.Its pathogenesis is that the evil lies in the interior,and the treatment is to penetrate the evil.The joint symptoms should belong to the category of damp-heat arthralgia of febrile disease,and the treatment should focus on“removing dampness”.According to the syndrome differentiation of Sanjiao,when treating arthralgia syndrome,Shangjiao should pay attention to opening lung qi,promoting smooth qi,Zhongjiao should be invigorating spleen to remove dampness,and Xiajiao should be tonifying liver and kidney to strengthen tendon and bone.This article will explore its application in the skin lesions and arthritis of psoriatic arthritis from these angles,understand psoriatic arthritis from the perspective of traditional Chinese medicine,and enriching the clinical diagnosis and treatment perspective.
