摘要
目的:比较长短InterTan髓内钉治疗不稳定型股骨转子间骨折的临床效果。方法:回顾性分析2017年1月-2019年10月安徽医科大学附属合肥医院收治并获得随访的61例不稳定型股骨转子间骨折患者资料。根据髓内钉主钉长度分为短钉组(n=30)和长钉组(n=31)。比较两组手术时间、术中出血量、围手术期输血率、并发症发生率、骨折愈合时间、髋关节Harris评分。结果:短钉组手术时间为(56.7±15.3)min,围手术期输血率为46.7%,长钉组手术时间为(68.6±18.1)min,围手术期输血率为74.2%,两组手术时间、围手术期输血率比较,差异均有统计学意义(P<0.05)。61例患者均获得随访,随访时间12~37个月,平均(19.3±6.8)个月,两组随访时间比较,差异无统计学意义(P>0.05)。两组术中出血量、围手术期并发症发生率、骨折愈合时间及末次随访髋关节Harris评分比较,差异均无统计学意义(P>0.05)。结论:对于骨折线延伸至小转子下方的不稳定型股骨转子间骨折,采用长短InterTan髓内钉固定均可获得满意疗效,而短钉固定的手术时间短、出血量少、输血率低。
Objective:To compare the clinical outcomes of short and long InterTan intramedullary nailing of unstable intertrochanteric femur fractures.Method:From January 2017 to October 2019,data of 61 patients with unstable intertrochanteric femur fractures in Hefei Hospital Affiliated to Anhui Medical University were retrospectively analyzed.Patients were divided into the short nail group(n=30)and the long nail group(n=31)according to the length of main staple of intramedullary nail.The operation time,intraoperative bleeding volume,perioperative blood transfusion rate,incidence of complication rate,fracture healing time,and Harris hip joint score were compared between the two groups.Result:In the short nail group,the operative time was(56.7±15.3)min,the perioperative blood transfusion rate was 46.7%.In the long nail group,the operative time was(68.6±18.1)min,the perioperative blood transfusion rate was 74.2%.The differences in operation time and perioperative blood transfusion rate between the two groups were statistically significant(P<0.05).All 61 patients were followed up for 12-37 months,with an average of(19.3±6.8)months,there was no significant difference in follow-up time between the two groups(P>0.05).There was no significant difference in intraoperative bleeding volume,incidence of perioperative complication rate,fracture healing time or Harris hip score at the last follow-up between the two groups(P>0.05).Conclusion:Either short or long InterTan intramedullary nailing is effective in treatment of unstable intertrochanteric femur fractures with fracture lines extending below the lesser trochanter.However,patients treated with short nails has shorter operative time,less blood loss,and low blood transfusions rate,which is better than the long nails.
