摘要
目的研究盐酸氨溴索在慢性鼻窦炎伴鼻息肉鼻内镜术后的应用效果。方法选取2017年1月至2020年1月南京医科大学附属淮安第一医院210例慢性鼻窦炎伴鼻息肉患者为研究对象,按照随机数字表法分为观察组和对照组,每组各105例。两组均行鼻窦内镜手术,对照组术后予常规糖皮质激素,观察组在对照组基础上使用盐酸氨溴索,两组均持续1个月。比较两组临床疗效,比较两组治疗前后症状评分和血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)水平,记录1年内两组鼻息肉复发情况。结果治疗后,观察组总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗后两组鼻塞、流涕、嗅觉障碍评分均较治疗前降低,且观察组显著低于对照组,差异有统计学意义(P<0.05)。治疗后两组PCT、hs-CRP、IL-6水平均较治疗前显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05)。治疗后1年观察组鼻息肉复发率明显低于对照组,差异有统计学意义(P<0.05)。结论盐酸氨溴索治疗慢性鼻窦炎伴鼻息肉效果较好,可改善鼻塞等多种症状,降低鼻息肉复发风险。
Objective To study the application effect of Ambroxol Hydrochloride in nasal endoscopy after chronic sinusitis with nasal polyp.Methods A total of 210 patients with chronic sinusitis complicated with nasal polyps in the Affiliated Huaian NO.1 People’s Hospital of Nanjing Medical University from January 2017 to January 2020 were selected as the research subjects,and they were divided into observation group and control group according to random number table method,with 105 cases in each group.Both groups received endoscopic sinus surgery,the control group received conventional glucocorticoids after surgery,and the observation group was additionally given Ambroxol Hydrochloride,Both groups lasted for one month.The clinical efficacy of the two groups was compared.Symptom scores and serum levels of procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP)and interleukin-6(IL-6)were compared between the two groups before and after treatment.The recurrence of nasal polyp in two groups was recorded within one year.Results After treatment,the total effective rate of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05).After treatment,the scores of nasal obstruction,runny nose,and olfactory disorder in two groups were lower than those before treatment,and the observation group was significantly lower than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of PCT,hs-CRP,and IL-6 in two groups were significantly decreased compared with before treatment,and the observation group was significantly lower than the control group,the differences were statistically significant(P<0.05).One year after treatment,the recurrence rate of nasal polyp in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Ambroxol Hydrochloride is effective in the treatment of chronic sinusitis complicated with nasal polyp,which can improve nasal congestion and other symptoms and reduce the risk of nasal polyp recurrence.
