摘要
目的:基于倾向性匹配评分法评估保乳手术、改良根治术两种术式治疗乳腺癌的效果及预后。方法:选取2015年1月至2020年12月收治的早期乳腺癌患者为研究对象,应用倾向性评分法分析保乳手术(A组)、改良根治术(B组)治疗早期乳腺癌患者的临床效果及预后。结果:共采集早期乳腺癌患者122例,倾向性评分法成功匹配41对;匹配后的两组随访时长、局部复发率及无复发生存时间比较差异无统计学意义(P>0.05),A组术中失血量显著低于对照组,切口长度、手术时间、住院时间显著短于对照组;术后A组乳腺癌人群的特异性问卷(QLQ-BR23)体象、性功能、性愉悦、对未来的期待条目分值高于B组;A组乳房症状、上肢症状评分则显著低于B组(P<0.05)。结论:早期乳腺癌患者行保乳手术治疗或可取得与改良根治术相当的生存获益,且保乳术切口长度短、术后恢复快、更利于维持良好的生存质量。
Objective:To evaluate efficacy and outcomes of breast conserving surgery vs modified radical mastectomy for breast cancer based on propensity score matching.Methods:Patients with early-stage breast cancer admitted between January 2015 and December 2020 were included as study subjects.The clinical efficacy and outcomes of breast conserving surgery(group A)and modified radical mastectomy(group B)for treatment of early-stage breast cancer were analyzed by propensity score matching.Results:A total of 122 patients with earlystage breast cancer were recruited.Propensity score matching was successful in 41 patients-pairs.There were no significant differences in length of follow-up,local recurrence and recurrence-free survival between the two groups after matching(P>0.05).The patients in group A experienced significantly less or shorter intraoperative blood loss,incision,operation time,and length of hospital stay than those in the control group.After surgery,the patients in group A scored significantly higher for body image,sexual function,sexual enjoyment,and future perspective,as well as lower for breast and arm symptoms by Quality of Life Questionnaire-Breast Cancer specific(QLQ-BR23),compared with those in group B(P<0.05).Conclusion:For patients with early-stage breast cancer,breastconserving surgery may achieve in survival benefit comparable to modified radical mastectomy.In addition,breastconserving surgery leads to shorter incision,earlier postoperative recovery,and is more conducive to maintaining a good quality of life.
