摘要
目的:探讨扶正化瘀胶囊联合异甘草酸镁对HBV感染失代偿期肝硬化患者的肝功能、免疫功能、血清炎症因子的影响。方法:选取HBV感染失代偿期肝硬化患者78例,按照随机数字表法分为对照组和病例组,每组39例。对照组给予异甘草酸镁治疗,病例组在对照组基础上给予扶正化瘀胶囊。比较两组疗效及治疗前后肝功能指标、肝纤维化指标、门静脉系统血流动力学指标、免疫指标、血清炎性因子。结果:病例组总有效率高于对照组(P<0.05)。治疗后,两组丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、总胆红素(TBIL)、透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(CⅣ)、门静脉内径(Dpv)、脾静脉内径(Dsv)、CD8^(+)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)较治疗前均降低,胆碱酯酶(CHE)、白蛋白(ALB)、门静脉血流速度(Vpv)、门静脉血流量(Qpv)、脾静脉血流速度(Vsv)、脾静脉血流量(Qsv)、CD3^(+)、CD4^(+)较治疗前均升高,且病例组肝功能、肝纤维化指标、门静脉系统血流动力学指标、免疫指标、血清炎症因子改善情况均优于对照组,差异均有统计学意义(P<0.05)。结论:扶正化瘀胶囊联合异甘草酸镁对HBV感染失代偿期肝硬化临床疗效显著,可以改善患者的肝功能,降低炎症反应程度,提高免疫状态。
Objective:To explore the effects of Fuzheng Huayu Capsule combined with Magnesium Isoglycyrrhizinate on liver function,immune function and serum inflammatory factors.Method:A total of 78 patients with liver cirrhosis during decompensated hepatitis were selected,they were divided into the control group and the case group,39 cases in each group.The control group was given Magnesium Isoglycyrrhizinate treatment,and the case group was given Fuzheng Huayu Capsule on the basis of the control group.The curative effects and liver function indexes,liver fibrosis indexes,hemodynamic indexes of portal vein system,immune indexes and serum inflammatory factors before and after treatment were compared between two groups.Result:The effective rate of the case group was higher than that of the control group (P<0.05).After treatment,alanine aminotransferase (ALT),aspartate aminotransferase (AST),total bilirubin (TBIL),hyaluronic acid (HA),laminin (LN),type Ⅲ procollagen(PCⅢ),type Ⅳ collagen (CⅣ),portal vein diameter (Dpv),splenic vein diameter (Dsv),CD8^(+),interleukin-6 (IL-6),interleukin-8 (IL-8),C-reactive protein (CRP),tumor necrosis factor-α (TNF-α) of two groups were lower than those before treatment,cholinesterase (CHE),albumin (ALB),portal vein blood flow (Vpv),portal vein blood flow (Qpv),splenic vein flow velocity (Vsv),splenic vein blood flow (Qsv),CD3^(+),CD4^(+)of two groups were higher than those before treatment,and the improvement of liver function,liver fibrosis index,portal venous system hemodynamics index,immune index,serum inflammatory factors of the case group were better than those of the control group,the difference was statistically significant (P<0.05).Conclusion:Fuzheng Huayu Capsule combined with Magnesium Isoglycyrrhizinate for patients with liver cirrhosis during decompensated hepatitis has a significant clinical effect,it can significantly liver fibrosis indicators and portal hemodynamics,reduce the degree of low inflammation,and improve the immune status.
