摘要
目的探究白细胞介素17(IL-17)在2型糖尿病患者血清、房水中表达水平及临床意义。方法方便选取2017年1月—2019年12月该院收治的白内障超声乳化联合人工晶体植入术或玻璃体切割术的45例2型糖尿病视网膜病变患者作为研究对象,所有患者接受眼底检查、荧光血管造影,依据检查结果分为糖尿病无视网膜病变期(NDR)组15例、非增殖期糖尿病视网膜病变(NPDR)组15例、增殖期糖尿病视网膜病变(PDR)组15例,选取同期收治的15例单纯老年性白内障患者作为对照组,分析各组IL-17在血清、房水中表达水平。结果4组生化指标比较,NDR组、NPDR组、PDR组高密度脂蛋白低于对照组,空腹血糖、糖化血红蛋白、总胆固醇、三酰甘油、低密度脂蛋白、尿素氮、肌酐高于对照组,差异有统计学意义(P<0.05);NDR组、NPDR组、PPR组血清IL-17水平[(33.26±3.26)、(42.26±4.46)、(48.59±4.96)pg/mL]高于对照组(15.59±2.26)pg/mL;房水IL-17水平[(22.26±2.26)、(26.56±3.26)、(30.26±3.69)pg/mL]高于对照组(10.26±2.63)pg/mL;血清IL-23水平[(22.26±5.59)、(45.26±3.56)、(64.56±4.56)pg/mL]高于对照组(14.56±2.26)pg/mL;血清TNF-α[(16.13±2.05)、(45.65±6.56)、(55.65±7.78)ng/L]高于对照组(4.62±1.26)ng/L;血清IL-6[(98.56±12.26)、(159.96±14.56)、(178.96±15.11)ng/L]高于对照组(65.26±8.59)ng/L,4组比较差异有统计学意义(P<0.05)。NDR组空腹血糖、糖化血红蛋白、总胆固醇、三酰甘油、低密度脂蛋白、尿素氮、肌酐、IL-23、TNF-α、IL-6低于NPDR组、PDR组,高密度脂蛋白水平高于NPDR组、PDR组,差异有统计学意义(P<0.05);NDR组血清IL-17、房水IL-17水平低于NPDR组、PDR组,差异有统计学意义(t=204.940、124.880,P<0.05)。血清IL-17与空腹血糖、糖化血红蛋白、总胆固醇、高密度脂蛋白无相关性(r=0.456、0.425、0.245、0.015,P>0.05);房水IL-17与空腹血糖、糖化血红蛋白、总胆固醇、高密度脂蛋白无相关性(r=0.568、0.154、0.265、0.016,P>0.05);血清IL-17与三酰甘油、低密度脂蛋白、尿素氮、肌酐、IL-23、TNF-α、IL-6具有相关性(r=-0.045、-0.025、-0.596、-0.598、0.598、0.485、0.369,P<0.05);房水IL-17与三酰甘油、低密度脂蛋白、尿素氮、肌酐、IL-23、TNF-α、IL-6具有相关性(r=-0.015、-0.015、-0.896、-0.789、-0.585、-0.598、-0.425,P<0.05)。结论血清和房水IL-17水平高表达可协同促进DR发生发展,与三酰甘油、低密度脂蛋白、尿素氮、肌酐具有相关性,为DR早期诊断和治疗提供新的靶点。
Objective To explore the expression level of interleukin 17(IL-17)in serum and aqueous humor of patients with type 2 diabetes and its clinical significance.Methods From January 2017 to December 2019,45 patients with type 2 diabetic retinopathy who received phacoemulsification combined with intraocular lens implantation or vitrectomy in the hospital were conveniently selected as the research objects.All patients underwent fundus examination and fluorescein angiography.According to the examination results,they were divided into 15 cases in diabetic non-retinopathy(NDR)group,15 cases in non-proliferative diabetic retinopathy(NPDR)group,and 15 cases in proliferative diabetic retinopathy(PDR)group.Selected 15 simply senile cataract patients with income during the same period as the control group.Analyzed the expression level of IL-17 in serum and aqueous humor of each group.Results Compared the biochemical indicators of the four groups,the fasting blood glucose of the NDRgroup,NPDR group,and PDR group were lower than those of the control group,the high-density,glycosylated hemoglobin,total cholesterol,triacylglycerol,low-density lipoprotein,urea nitrogen,and creatinine were higher than those of the control group,the difference was statistically significant(P<0.05);the serum IL-17 level of the NDR group,NPDR group,PDR group[(33.26±3.26)pg/mL,(42.26±4.46)pg/mL,(48.59±4.96)pg/mL]were igher than the control group(15.59±2.26)pg/mL;aqueous IL-17 level[(22.26±2.26)pg/mL,(26.56±3.26)pg/mL,(30.26±3.69)pg/mL]were higher than the control group(10.26±2.63)pg/mL;serum IL-23 level[(22.26±5.59)pg/mL,(45.26±3.56)pg/mL,(64.56±4.56)pg/mL]was higher than the control group(14.56±2.26)pg/mL;serum TNF-α[(16.13±2.05)ng/L,(45.65±6.56)ng/L,(55.65±7.78)ng/L]were higher than the control group(4.62±1.26)ng/L;serum IL-6[(98.56±12.26)ng/L,(159.96±14.56)ng/L,(178.96±15.11)ng/L]were higher than the control group(65.26±8.59)ng/L,the difference between the four groups was statistically significant(P<0.05).Fasting blood glucose,glycosylated hemoglobin,total cholesterol,triglyceride,low density lipoprotein,urea nitrogen,creatinine,IL-23,TNF-α,IL-6 in NDR group and PDR group were lower than those in NPDR group and PDR group.HDL levels higher than NPDR group and PDR group,the difference was statistically significant(P<0.05).The levels of serum IL-17 and aqueous IL-17 in NDR group were lower than those in NPDR group and PDR group,the difference was statistically significant(t=204.940,124.880,P<0.05).There was no correlation between serum IL-17 and fasting blood glucose,HbA1c,total cholesterol and high-density lipoprotein(r=0.456,0.425,0.245,0.015,P>0.05).Aqueous humor IL-17 and fasting glucose,glycosylated hemoglobin,total cholesterol,high-density lipoprotein cholesterol(HDL-C)no correlation(r=0.568,0.154,0.265,0.016,P>0.05).Serum IL-17 and three acyl glycerin,low density lipoprotein cholesterol(LDL-C),urea nitrogen,creatinine,IL-23,TNF-α,IL-6 had correlation(r=0.045,0.025,0.596,0.598,0.598,0.485,0.369,P<0.05).Aqueous humor IL-17 and three acyl glycerin,low density lipoprotein cholesterol(LDL-C),urea nitrogen,creatinine,IL-23,TNF-α,IL-6,had correlation(r=-0.015、-0.015、-0.896、-0.789、-0.585、-0.598、-0.425,P<0.05).Conclusion The high expression of IL-17 in serum and aqueous humor can synergistically promote the occurrence and development of DR,and is correlated with triacylglycerol,low-density lipoprotein,urea nitrogen,and creatinine,providing new targets for early diagnosis and treatment of DR.
