摘要
目的研究加速康复外科(ERAS)护理理念对皮瓣移植术治疗的口腔癌患者术后营养状况、康复情况的改善效果。方法选取2017年5月至2020年5月在蚌埠医学院第一附属医院接受皮瓣移植术治疗的口腔癌患者100例,按随机数字表法分为观察组和对照组,每组各50例,对照组采用常规护理,观察组采用ERAS护理。比较两组患者术后营养指标、康复指标以及炎性指标的差异。结果术后14 d,观察组血清白蛋白、转铁蛋白、前白蛋白、血红蛋白水平高于对照组(P<0.05);观察组IL-6、CRP、TNF-α水平低于对照组(P<0.05)。观察组术后并发症总发生率低于对照组(P<0.05);观察组术后下床活动时间、输液时间、住院时间短于对照组,焦虑得分低于对照组(P<0.05)。结论加速康复外科护理理念在口腔癌行皮瓣移植术患者中的应用,能有效改善患者术后营养状况,促进患者术后快速康复,改善患者体内炎症反应,具有较高价值。
Objective To study the effects of the nursing concept of enhanced recovery after surgery(ERAS)on the improvement of postoperative nutritional status and recovery of patients undergoing flap transplantation for oral cancer.Methods A total of 100 patients with oral cancer undergoing flap transplantation in the First Affiliated Hospital of Bengbu Medical College from May 2017 to May 2020 were screened and divided into the observation group and the control group by random number table method,with 68 cases in each group.The control group received routine nursing,and the observation group received ERAS nursing.the differences of postoperative nutritional indexes,rehabilitation indexes and inflammatory indexes between the two groups were compared.Results At 14 d after surgery,the levels of serum albumin,transferrin,prealbumin,and hemoglobin in the observation group were higher than those in the control group(P<0.05).The levels of interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).The total incidence of postoperative complications in the observation group(16.00%)was lower than(52.00%)in the control group(P<0.05).The postoperative ambulation time,transfusion time,and hospital stay in the observation group were shorter than those in the control group,and the anxiety score in the observation group was lower than that in the control group(P<0.05).Conclusion The application of the ERAS nursing concept in patients undergoing flap transplantation for oral cancer can effectively improve postoperative nutritional status,promote rapid postoperative recovery and improve in vivo inflammatory response of patients,which has higher value.
