摘要
目的:探讨加味四妙勇安汤联合艾灸治疗类风湿性关节炎(RA)的疗效及对患者病情活动指标的影响。方法:选取100例湿热痹阻型RA患者,采用简单随机分组法将患者分为观察组和对照组,每组各50例。对照组采用艾灸治疗,观察组在对照组基础上联合加味四妙勇安汤治疗。评估两组临床疗效和治疗期间的不良反应发生情况。分别于治疗前后检测患者中医证候积分、压痛关节数(TJC)、肿胀关节数(SJC)、晨僵时间、DAS28评分、VAS评分、红细胞沉降率(ESR)、血小板计数(PLT)、C-反应蛋白(CRP)、类风湿因子(RF)水平。结果:观察组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者中医证候积分、SJC和TJC数量、晨僵时间、DAS28评分、VAS评分、ESR、CRP、PLT、RF水平均低于对照组,差异有统计学意义(均P<0.05)。治疗期间,两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:加味四妙勇安汤联合艾灸治疗RA临床疗效较好,可缓解关节肿痛,缩短晨僵时间,减轻炎症反应,安全性较好。
Objective:To investigate the curative effect of modified Simiao Yongan decoction combined with moxibustion on rheumatoid arthritis(RA)and its influences on disease activity indexes.Methods:100 patients with RA of Shire Bizu type were divided into observation group and control group according to simple random grouping method,50 cases in each group.The control group was treated with moxibustion,while the observation group was additionally treated with modified Simiao Yongan decoction.The clinical curative effect and occurrence of adverse reactions during treatment in both groups were evaluated.The levels of Chinese medicine syndrome score,tender joint count(TJC),swollen joint count(SJC),morning stiffness time,DAS28 score,VAS score,erythrocyte sedimentation rate(ESR),platelet(PLT),C-reactive protein(CRP),rheumatoid factor(RF)were determined before and after treatment.Results:The response rate of treatment in the observation group was significantly higher than that in the control group,difference statistically significant(P<0.05).After treatment,scores of TCM syndromes,SJC,TJC,morning stiffness time,DAS28 and VAS scores,and levels of ESR,CRP,PLT and RF in the observation group were significantly lower than those in the control group,difference statistically significant(all P<0.05).The incidence of adverse reactions during treatment between the two groups was not statistically significant(P>0.05).Conclusion:Modified Simiao yongan decoction combined with moxibustion has good clinical efficacy in the treatment of RA,which can relieve joint swelling and pain,shorten the time of morning stiffness,relieve inflammatory reaction,with better safety.
