摘要
目的分析机采血小板采集失败的影响因素,并针对原因提出改进措施。方法回顾分析2016年6月—2021年6月河源市中心血站机采血小板献血者的临床资料,选择献血失败的22例献血者作为研究对象,另外选择同期于该站采血成功的100名献血者作为对照组。检测所有献血者各项外周血红细胞平均值参数;采用Logistic回归方程分析机采血小板采集失败的影响因素,并针对失败因素提出解决对策。结果机采血小板采集失败的22例献血者中,31.82%(7/22)的献血者因枸椽酸反应所致,22.73%(5/22)的献血者因血小板冲红所致,18.18%(4/22)的献血者因精神因素(过度紧张)所致,13.64%(3/22)的献血者因穿刺失败所致,9.09%(2/22)的献血者因采血工作人员操作不当所致,4.54%(1/22)的献血者由其他因素所致。经Logistic回归分析显示,出现枸椽酸反应〔优势比(OR)=1.225,95%可信区间(95%CI)为1.059~2.331,P=0.001〕、冲红现象(OR=1.632,95%CI为1.011~2.399,P=0.001)、耗材不合格(OR=1.985,95%CI为1.003~2.058,P=0.008)均为导致机采血小板采集失败的危险因素。结论机采血小板采集失败常因献血者自身因素以及工作人员不合理操作所致,因此,通过加强采集前检测及献血者筛选,同时规范工作人员操作规程,能有效避免机采血小板采集失败,提高采集成功率。
Objective To analyze the influence factors for the failure of fresh apheresis platelet collection by machine and put forward the corresponding improvement measures.Methods The clinical data of blood donors receiving the fresh apheresis platelet collection by machine from June 2016 to June 2021 in Heyuan Central Blood Station were retrospectively analyzed.The 22 failed cases of fresh apheresis platelet collection by machine were selected as research subjects,and over the same time period,100 successful cases were selected as control group.The morphological indexes of red blood cells were measured;Logistic regression equation was used to analyze the influence factors for the failure of fresh apheresis platelet collection by machine and the corresponding solution measures were put forward.Results Among the 22 failure cases of fresh apheresis platelet collection by machine,31.82%(7/22)of donors were caused by citric acid reactions,22.73%(5/22)of them were caused by pulsed blood platelet,18.18%(4/22)of them were caused by mental factors,13.64%(3/22)of them were caused by puncture errors,9.09%(2/22)of them were caused by misoperation and 4.54%(1/22)of them were caused by other reasons.Logistic regression equation analysis showed that citric acid reactions[odds ratio(OR)=1.225,95%confidence interval(95%CI)was 1.059-2.331,P=0.001],pulsed blood platelet(OR=1.632,95%CI was 1.011-2.399,P=0.001)and unqualified consumadles(OR=1.985,95%CI was 1.003-2.058,P=0.008)were the risk factors for failure of fresh apheresis platelet collection by machine.Conclusions The failure of platelet collection by machine is often caused by the blood donors'own factors and the unreasonable operation of the staff.Therefore,by strengthening the detection before collection,screening the blood donors,and standardizing the operating procedures of the staff,we can effectively avoid the failure of platelet collection by machine and improve the success rate of collection.
