摘要
目的系统评价冠状动脉(冠脉)侧支循环影响因素。方法计算机检索Web of Science、The Cochrane Library、PubMed、EMbase、CNKI、WanFang及CBM数据库,搜集建库至2020年1月有关冠脉侧支循环相关影响因素的病例对照研究。由2名研究者独立阅读并筛选文献,提取资料并评价纳入研究的偏倚风险后,使用RevMan 5.3软件进行Meta分析。结果共纳入22个研究,包括6186例患者。本研究结果显示:糖尿病(OR=1.49,95%CI:1.20~1.85,P=0.0002)、C反应蛋白(MD=1.59,95%CI:0.11~3.08,P=0.04)、白细胞(MD=0.81,95%CI:0.62~1.00,P<0.00001)、中性粒细胞/淋巴细胞比值(MD=1.53,95%CI:0.54~2.51,P=0.002)、三酰甘油(MD=0.12,95%CI:0.00~0.24,P=0.04)、病变血管数(MD=-0.19,95%CI:-0.35~-0.03,P=0.02)、口服他汀药物(OR=0.80,95%CI:0.69~0.92,P=0.002)是冠脉侧支循环影响因素。结论当前证据表明,糖尿病、C反应蛋白、白细胞、中性粒细胞/淋巴细胞比值、三酰甘油、病变血管数、口服他汀药物均是冠脉侧支循环影响因素。受纳入研究的数量和质量的局限,需要更多研究验证结论。
Objective To review systematically the influencing factors of coronary collateral circulation.Methods The databases of Web of Science,Cochrane Library,PubMed,EMbase,CNKI,WanFang Data and CBM were retrieved with computer until Jan.2020 to collect the case-control studies related to the influencing factors of coronary collateral circulation.The literature was read and screened,data was extracted and bias risk of included studies was reviewed by 2 researchers independently,and a Meta-analysis was conducted by using RevMan 5.3 software.Results There were totally 22 studies included involved 6186 patients.The results of the analysis showed that diabetes(OR=1.49,95%CI:1.20~1.85,P=0.0002),C-reactive protein(MD=1.59,95%CI:0.11~3.08,P=0.04),white blood cell(MD=0.81,95%CI:0.62~1.00,P<0.00001),neutrophil/lymphocyte ratio(MD=1.53,95%CI:0.54~2.51,P=0.002),triglyceride(MD=0.12,95%CI:0.00~0.24,P=0.04),number of diseased vessels(MD=-0.19,95%CI:-0.35~-0.03,P=0.02)and oral statins(OR=0.80,95%CI:0.69~0.92,P=0.002)were influencing factors of coronary collateral circulation.Conclusion The current evidence indicates that diabetes,C reactive protein,white blood cells,neutrophil/lymphocyte ratio,triglyceride,number of diseased vessels and oral statins are influencing factors of coronary collateral circulation.As the limitations of the quantity and quality of included studies,more studies are needed to verify the conclusion.
