白内障超声乳化术中能量输出与术后疗效的关系探讨 被引量:1

摘要 目的探讨白内障超声乳化时能量输出与手术后恢复情况的关系并分析其影响因素。方法选取2019年3月至2020年10月本院白内障科收治的不同类型和核硬度的单纯性年龄相关性白内障患者112例(174眼),所有患者均接受白内障超声乳化手术联合人工晶状体植入,分析术中能量输出与年龄、性别、术后角膜情况、眼压以及术后视功能之间的关系。结果白内障患者术中累计释放能量(CDE)为(5.80±16.47),超声平均时间为(13.52±15.38)s。术后第1天角膜评分(0.21±1.34)分,术后第1天患者视力(0.62±0.49),最佳矫正视力≥1.0者78眼(44.8%),≥0.5者81眼(46.6%),0.1~0.25者10眼(5.7%),<0.1者5眼(2.9%)。术后第1天眼压(14.56±8.41)mmHg,超出正常眼压患者11眼,最高29.4 mmHg,最低6.7 mmHg,与术前眼压比较差异无统计学意义(t=0.37,P=0.74)。出现前房反应者8眼(6.3%),程度均较轻,未发现感染性眼内炎患者。结论超声乳化手术过程中的超声能量输出与患者术后角膜情况密切相关,直接影响白内障术后患者的短期临床效果。 Objective To explore the relationship between energy output during phacoemulsification and recovery after surgery,and to analyze its influencing factors.Methods 112 patients with 174 eyes of simple age-related cataract of different types and nuclear hardness who were diag-nosed in the Cataract Department of our hospital from March 2019 to October 2020 were selected.All patients underwent phacoemulsification and in-traocular lens implantation.Analyzed the relationship between intraoperative energy output and age,gender,postoperative corneal conditions,intraoc-ular pressure,and postoperative visual function recovery.Results The cumulative dissipated energy(CDE)of cataract patients was(5.80±16.47),and the average ultrasound output time was(13.52±15.38)s.The corneal score was(0.21±1.34)scores on the first postoperative day,the visual acuity level of the patient was(0.62±0.49)on the first postoperative day,78 eyes(44.8%)with best corrected visual acuity≥1.0,81 eyes(46.6%)≥0.5,0.1-0.25 had 10 eyes(5.7%),5 eyes(2.9%)were<0.1.On the first postoperative day,the intraocular pressure was(14.56±8.41)mmHg,11 eyes of pa-tients with high intraocular pressure,the highest was 29.4 mmHg,the lowest was 6.7 mmHg,there was no difference with intraocular pressure before operation(t=0.29,P=0.74).There were 8 eyes(4.60%)with anterior chamber reaction,all of which were mild,and no patients with infectious endo-phthalmitis were found.Conclusion The ultrasound energy output during phacoemulsification is closely related to the postoperative corneal condi-tion,which directly affects the short-term clinical effect of patients after cataract surgery.
