摘要
目的:探讨子宫肌瘤患者在应用西药治疗的基础上加用少腹逐瘀汤加减后的临床效果,同时观察患者治疗后的血流动力学及激素水平变化。方法:将2018年2月-2020年9月在辽宁省海城市中心医院妇科治疗的88例子宫肌瘤患者随机分为对照组和观察组,每组44例。对照组口服米非司酮片,观察组口服米非司酮片的同时加用中药少腹逐瘀汤加减,两组均连续用药3个月。比较两组临床疗效,治疗前后血流动力学[舒张末期血流速峰值(D)、收缩末期血流速峰值(S)、搏动指数(PI)和阻力指数(RI)]、子宫体积、子宫肌瘤体积及性激素[血清卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E;)和孕酮(P)]水平。结果:观察组总有效率为93.18%,高于对照组的77.27%(P<0.05)。治疗前,两组血流动力学、子宫体积、子宫肌瘤体积及性激素水平比较,差异均无统计学意义(P>0.05)。治疗后,两组PI和RI较治疗前均升高,且观察组均高于对照组(P<0.05);治疗后,两组S和D较治疗前均降低(P<0.05),观察组S明显低于对照组(P<0.05),但两组D比较差异无统计学意义(P>0.05)。治疗后,两组子宫体积和子宫肌瘤体积与治疗前比较均降低(P<0.05),且观察组均小于对照组(P<0.05)。治疗后,两组血清FSH、LH、E;和P较治疗前均降低(P<0.05),且观察组均低于对照组(P<0.05)。结论:少腹逐瘀汤加减联合西药治疗子宫肌瘤可有效改善患者临床症状和血流动力学指标,且性激素水平显著降低,效果明显。
Objective:To explore the clinical effect of Shaofu Zhuyu Decoction on the basis of Western medicine in patients with hysteromyoma,and to observe the changes of hemodynamics and hormone levels after treatment.Method:A total of 88 patients with hysteromyoma treated in Haicheng Central Hospital from February 2018 to September 2020 were randomly divided into the control group and the observation group,44 cases in each group.The control group was treated with Mifepristone Tablets,and the observation group was treated with Shaofu Zhuyu Decoction on the basis of the control group,two groups were treated continuously for 3 months.The clinical efficacy,hemodynamics[peak diastolic blood flow(D),peak systolic blood flow(S),pulsatility index(PI)and resistance index(RI)],uterine volume,uterine leiomyoma volume and the levels of sex hormones[serum follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E;)and progesterone(P)]before and after treatment were compared between two groups.Result:The total effective rate in the observation group was 93.18%,which was higher than 77.27%in the control group(P<0.05).Before treatment,there were no significant differences in hemodynamics,uterine volume,uterine fibroid volume and sex hormone level between two groups(P>0.05).After treatment,PI and RI in two groups were higher than those before treatment,and the observation group were higher than those in the control group(P<0.05);after treatment,S and D in two groups were lower than those before treatment(P<0.05),S in the observation group was significantly lower than that in the control group(P<0.05),but there was no significant difference between two groups in D(P>0.05).After treatment,the uterus volume and hysteromyoma volume in two groups decreased compared with those before treatment(P<0.05),and the observation group were smaller than those in the control group(P<0.05).After treatment,serum FSH,LH,E;and P in two groups were lower than those before treatment(P<0.05),and the observation group were lower than those in the control group(P<0.05).Conclusion:Shaofu Zhuyu Decoction combined with western medicine in the treatment of hysteromyoma can effectively improve clinical symptoms and hemodynamic indexes the effect is obvious,reduce level of sex hormone,effect is obvious.
