摘要
目的:探讨用宫腔镜子宫内膜息肉电切术(TCRP)联合曼月乐宫内节育器治疗子宫内膜息肉的疗效。方法:选取2020年1月至2021年3月东莞市滨海湾中心医院收治的100例子宫内膜息肉患者作为研究对象。采用随机数表法将其分为试验组(n=50)与对照组(n=50)。为对照组患者采用TCRP进行治疗,在此基础上为试验组患者加用曼月乐宫内节育器进行治疗,然后比较两组患者的临床疗效、子宫内膜的厚度、月经失血图(PBAC)的评分、血红蛋白的水平和病情复发的情况。结果:试验组患者治疗的总有效率高于对照组患者,P<0.05。治疗6个月后,两组患者血红蛋白的水平均较治疗前显著升高,其子宫内膜的厚度均较治疗前显著减小,其PBAC评分均较治疗前显著降低,P<0.05;试验组患者血红蛋白的水平高于对照组患者,其子宫内膜的厚度小于对照组患者,其PBAC评分低于对照组患者,P<0.05。在随访期间,试验组患者并发症的发生率、病情的复发率均低于对照组患者,P<0.05。结论:用TCRP联合曼月乐宫内节育器治疗子宫内膜息肉的疗效确切,能够促使患者的子宫内膜变薄,促进其月经量恢复正常,提高其血红蛋白的水平,防止其病情复发。
Objective:To investigate the efficacy of hysteroscopic electroresection of endometrial polyp(TCRP)combined with Mirena intrauterine device in the treatment of endometrial polyp.Methods:a total of 100 cases of patients with intrauterine polyps admitted to Dongguan Binhai Bay Central Hospital from January 2020 to March 2021 were selected as subjects.They were divided into experimental group(n=50)and control group(n=50)by random number table method.Patients in the control group were treated with TCRP,and patients in the experimental group were treated with mirena intrauterine device on this basis.Then,the clinical efficacy,endometrial thickness,menstrual blood loss map(PBAC)score,hemoglobin level and disease recurrence were compared between the two groups.Results:The total effective rate of experimental group was higher than that of control group,P<0.05.After 6 months of treatment,the level of hemoglobin in 2 groups was significantly increased,the thickness of endometrium was significantly decreased,and the PBAC score was significantly decreased,P<0.05;The hemoglobin level of the experimental group was higher than that of the control group,the endometrial thickness was lower than that of the control group,and the PBAC score was lower than that of the control group,P<0.05.During the follow-up period,the incidence of complications and recurrence rate in the experimental group were lower than those in the control group,P<0.05.Conclusion:TCRP combined with mirena intrauterine device is effective in the treatment of endometrial polyps,which can promote the endometrial thinning of patients,promote the normal menstrual volume,improve the level of hemoglobin,and prevent the recurrence of the disease.
