显微镜下直切口小骨窗经外侧裂入路颅内血肿清除术治疗基底节区脑出血的临床效果 被引量:4

Clinical effect of microscopically assisted evacuation of intracranial hematoma by straight incision small bone window through lateral fissure approach in the treatment of intracerebral hemorrhage in basal ganglia region

摘要 目的分析显微镜下直切口小骨窗经外侧裂入路颅内血肿清除术治疗基底节区脑出血的临床效果。方法选择2018年12月至2020年12月本院收治的基底节区脑出血患者80例,采用红绿双色球法随机将其分为对照组和试验组,每组40例。对照组给予传统大骨瓣手术治疗,试验组给予显微镜下直切口小骨窗经外侧裂入路颅内血肿清除术治疗。比较两组手术前、后的脑脊液炎症因子水平、神经功能与生活能力。结果手术后,试验组的hs-CRP、IGF-1、IL-6水平均低于对照组(P<0.05)。手术后2个月,试验组的NIHSS、ADL评分均低于对照组(P<0.05)。结论显微镜下直切口小骨窗经外侧裂入路颅内血肿清除术治疗基底节区脑出血患者的效果理想,能够改善炎症反应、神经功能与生活能力,临床可进一步推广。 Objective To analyze the clinical effect of microscopically assisted evacuation of intracranial hematoma by straight incision small bone window through lateral fissure approach in the treatment of intracerebral hemorrhage in basal ganglia region.Methods A total of 80 patients with intracerebral hemorrhage in basal ganglia region admitted in our hospital from December 2018 to December 2020 were selected and randomly divided into control group and experimental group by red-green bicolor method,with 40 cases in each group.The control group was treated with traditional large bone flap surgery,and the experimental group was treated with microscopically assisted evacuation of intracranial hematoma by straight incision small bone window through lateral fissure approach.The levels of cerebrospinal fluid inflammatory factors,neurological function and living ability were compared between the two groups before and after operation.Results After operation,the levels of hs-CRP,IGF-1 and IL-6 in the experimental group were lower than those in the control group(P<0.05).Two months after operation,the NIHSS and ADL scores of the experimental group were lower than those of the control group(P<0.05).Conclusion Microscopically assisted evacuation of intracranial hematoma by straight incision small bone window through lateral fissure approach in the treatment of patients with intracerebral hemorrhage in basal ganglia region has an ideal effect,which can improve inflammatory response,neurological function and living ability,and can be further popularized in clinic.
