摘要
目的探讨重组人干扰素α-1b联合地塞米松治疗重症手足口病合并病毒性脑膜炎的临床效果。方法选取重症手足口病合并病毒性脑膜炎患儿72例,按治疗方案不同分为3组:对照A组23例予以地塞米松治疗,对照B组23例予以重组人干扰素α-1b治疗,研究组26例予以重组人干扰素α-1b联合地塞米松治疗。比较3组临床疗效、症状消失时间(退热时间、皮疹消退时间)及治疗前后血清炎性因子及脑损伤因子[肿瘤坏死因子-α(TNF-α)、神经特异性烯醇化酶(NSE)、S-100B蛋白]水平。结果与对照A组和对照B组比较,研究组总有效率显著升高,退热时间、皮疹消退时间均显著缩短,差异有统计学意义(均P<0.05);与治疗前比较,3组治疗后血清TNF-α、NSE、S-100B蛋白水平显著降低,且研究组治疗后上述指标降低更为明显,差异有统计学意义(均P<0.05)。结论重组人干扰素α-1b联合地塞米松治疗重症手足口病合并病毒性脑膜炎患儿临床效果显著,可有效促使临床症状消退,且具有良好的脑保护作用,可有效缓解机体炎症反应,减轻脑损伤。
Objective To investigate the clinical effect of recombinant human interferonα-1b combined with dexamethasone on severe hand-foot-and-mouth disease complicated by viral meningitis.Methods A total of 72 children with severe hand-foot-and-mouth disease and viral meningitis were treated with dexamethasone(control group A,23 cases),recombinant human interferonα-1b(control group B,23 cases),or their combination(study group,26 cases).The clinical efficacy,time of symptom disappearance(defervescence time and rash remission time),and serum levels of tumor necrosis factor-α(TNF-α),neuron specific enolase(NSE)and S-100B protein were compared among the three groups.Results Compared with the control group A or B,the total effective rate was increased and defervescence time and rash remission time were shortened in the study group(P<0.05).After treatment,serum levels of TNF-α,NSE and S-100B protein significantly decreased in all the three groups,and the decrease in the study group was more obvious than that in the control group A or B(P<0.05).Conclusion Recombinant human interferonα-1b combined with dexamethasone can effectively promote the regression of clinical symptoms,alleviate inflammation and protect against brain injury in children with hand-foot-and-mouth disease complicated by viral meningitis.
