床边超声心动图监测脓毒症休克患者血容量对右心功能的影响 被引量:7

摘要 目的探讨床边超声心动图监测脓毒症休克患者血容量对右心功能的影响。方法选取2020年5~12月中国科技大学附属第一医院(安徽省立医院)南区收治的脓毒症休克患者80例,根据左室射血分数,分为左心功能正常组(n=40)与左心功能异常组(n=40),同期选择40例健康志愿者作为对照组,比较3组研究对象超声心动图检查指标,采用Pearson相关性分析脓毒症休克患者下腔静脉呼气末内径(IVCmax)、下腔静脉吸气末内径(IVCmin)、呼吸变异指数(SCI)与三尖瓣环收缩期位移(TAPSE)的相关性。结果左心功能异常组患者IVCmax为(21.55±2.30)mm、IVCmin为(12.28±3.52)mm,均高于对照组、左心功能正常组,TAPSE为(17.32±3.72)mm、SCI为(42.79±15.98)%,均低于对照组、左心功能正常组,差异有统计学意义(P<0.05);Person相关分析结果显示,左心功能正常组、左心功能异常组患者IVCmax与TAPSE均呈负相关(r=-0.610,P<0.001;r=-0.730,P<0.001),左心功能异常组SCI与TAPSE呈正相关(r=0.800,P<0.001)。结论脓毒症休克患者IVCmax、SCI与TAPSE密切相关,反映血容量变化对右心功能的影响。
