颈动脉彩色多普勒超声和超声造影微血管成像评估冠状动脉粥样硬化性心脏病患者颈动脉狭窄准确率的对比研究 被引量:6

摘要 目的探究颈动脉彩色多普勒超声(CDFI)和超声造影微血管成像(MVI)评估冠状动脉粥样硬化性心脏病(CHD)患者颈动脉狭窄的准确率。方法回顾性分析2019年1月至2020年12月我院收治的84例CHD患者(168条血管)的颈动脉CDFI和MVI影像学资料,以数字减影血管造影(DSA)检查为金标准,对比颈动脉CDFI和MVI评估对CHD患者颈动脉狭窄准确率。结果84例患者共168条血管,DSA检查按轻、中、重度及完全闭塞分别为51条、56条、61条,颈动脉CDFI检查按轻、中、重度及完全闭塞分别为56条、58条、54条;MVI检查按轻、中、重度及完全闭塞分别为51条、57条、60条。以DSA检查作为金标准,颈动脉CDFI诊断重度狭窄及完全闭塞的敏感度为78.7%,Kappa值为0.749;MVI诊断敏感度为96.7%,Kappa值为0.961。经颈动脉CDFI检查,轻度狭窄诊断率为82.2%,中度狭窄诊断率为94.6%,重度狭窄及完全闭塞诊断率为78.7%,总计为88.1%;经MVI检查,轻度狭窄诊断率为98.0%,中度狭窄诊断率为98.2%,重度狭窄及完全闭塞诊断率为96.7%,合计为97.6%,MVI诊断颈动脉狭窄程度准确率较高。结论MVI对于诊断颈动脉轻中-重度及以上的CHD患者诊断效能高于颈动脉CDFI。 Objective To explore the accuracy of carotid artery color Doppler flow imaging(CDFI)and contrast-enhanced ultrasound microvascular imaging(MVI)in assessing the carotid artery stenosis in patients with coronary atherosclerotic heart disease(CHD).Methods Retrospective analysis was performed on the imaging data of carotid artery CDFI and MVI of 84 patients with CHD(168 vessels)who were treated in the hospital between January 2019 and December 2020.With digital subtraction angiography(DSA)as the gold standard,the accuracy of carotid artery CDFI and MVI in assessing the carotid artery stenosis was compared among patients with CHD.Results In this study,there were a total of 168 blood vessels among 84 patients,including 51 vessels of mild stenosis,56 vessels of moderate stenosis and 61 vessels of severe stenosis and complete occlusion respectively by DSA examination.There were 56 vessels of mild stenosis,58 vessels of moderate stenosis and 54 vessels of severe stenosis and complete occlusion respectively by carotid artery CDFI examination,and there were 51 vessels of mild stenosis,57 vessels of moderate stenosis and 60 vessels of severe stenosis and complete occlusion by MVI examination.Taking DSA examination as the gold standard,the sensitivity and Kappa value were 78.7%and 0.749 of carotid artery CDFI and were 96.7%and 0.961 of MVI in the diagnosis of severe stenosis and complete occlusion.By carotid artery CDFI,the diagnosis rates of mild stenosis,moderate stenosis and severe stenosis and complete occlusion were 82.2%,94.6%and 78.7%,with 88.1%in total.By MVI examination,the diagnosis rates of mild stenosis,moderate stenosis and severe stenosis and complete occlusion were 98.0%,98.2%and 96.7%,with 97.6%in total.The accuracy rate of MVI was higher in diagnosing carotid artery stenosis.Conclusion MVI has higher diagnostic efficiency than carotid artery CDFI in diagnosing patients with mild,moderate to severe CHD and above.
