摘要
目的探讨胃癌术后静脉血栓栓塞(VTE)患者血清基质金属蛋白酶-9(MMP-9)、巨噬细胞移动抑制因子(MIF)、纤维蛋白单体(FM)水平变化及其临床意义。方法选择2017年1月至2020年1月绍兴文理学院附属医院胃癌术后VTE患者30例作为观察组,另选择60例单纯胃癌术后患者作为对照组。比较两组患者术后血清MMP-9、MIF、FM水平,分析与术后VTE发生的相关因素,评价血清MMP-9、MIF、FM水平对胃癌术后VTE的诊断价值及与患者预后的关系。结果观察组术后第3、5、7天血清MMP-9、MIF、FM均高于对照组,差异均有统计学意义(均P<0.05)。血清MMP-9、MIF、FM均是胃癌术后VTE的主要影响因素(均P<0.05)。三者联合诊断VTE的AUC最大,为0.962。MMP-9>348.49ng/mL、MIF>213.08ng/mL、FM>22.11μg/mL患者3个月病死率高于MMP-9≤348.49 ng/mL、MIF≤213.08 ng/mL、FM≤22.11μg/mL患者,差异均有统计学意义(均P<0.05)。结论血清MMP-9、MIF和FM在胃癌术后VTE患者中均增高,并与不良预后相关。
Objective To investigate the changes of serum matrix metalloproteinase-9(MMP-9),macrophage migration inhibitory factor(MIF)and fibrin monomer(FM)in patients with venous thromboembolism(VTE)after gastric cancer(GC)surgery and its clinical significance.Methods Thirty GC patients with post-operative VTE(observation group)and 60 GC patients without post-operative VTE(control group)in Affiliated Hospital of Shaoxing University of Arts and Sciences were selected from January 2017 to January 2020.Serum MMP-9,MIF,and FM were compared between the two groups and the factors related to the occurrence of post-operative VTE were analyzed.The value of serum MMP-9,MIF and FM in the diagnosis of post-operative VTE and its association with the prognosis of patients were evaluated.Results Serum MMP-9,MIF,and FMin the observation group were significantly higher than those in the control group on the 3rd,5th,and 7th day after operation(all P<0.05).Serum MMP-9,MIF,and FM were the main influencing factors of post-operative VTE(all P<0.05).The area under the curve(AUC)of combined MMP-9,MIF and FM in diagnosis of VTE was 0.962,higher than that of each parameter.The 3-month mortality was significantly higher in patients with MMP-9>348.49 ng/mL,MIF>213.08 ng/mL and FM>22.11μg/mL than in patients with MMP-9≤348.49 ng/mL,MIF≤213.08 ng/mL and FM≤22.11μg/mL group(all P<0.05).Conclusion SerumMMP-9,MIF and FMincrease in all GC patients with post-operative VTE,which is related to poor outcome.
