摘要
目的分析不同肿瘤分期(TNM)膀胱癌患者外周血循环肿瘤细胞(CTCs)的表达情况,并探讨其临床意义。方法选取空军军医大学唐都医院2018年6月至2020年9月收治的58例膀胱癌患者作为研究对象,所有患者均进行CTCs检测,比较不同临床病理特征(包括性别、病理类型、年龄、分化程度、肿瘤大小、TNM总分期、T分期、N分期、M分期、脉管侵犯及神经侵犯)患者的CTCs阳性情况以及不同TNM分期患者的CTCs计数情况。结果 58例患者中CTCs阳性者共22例;膀胱癌患者不同性别、病理类型、年龄、分化程度及肿瘤大小间的CTCs阳性率比较差异均无统计学意义(P>0.05);患者TNM总分期(Ⅲ期+Ⅳ期)、T_(3)+T_(4)、N1+N_(2)+N_(3)、M_(1)、脉管侵犯及神经侵犯患者中CTCs阳性率分别为50.00%、51.72%、53.13%、80.00%、46.34%、47.37%,明显高于Ⅰ期+Ⅱ期、T_(1)+T_(2)、N_(0)、M_(0)及无脉管侵犯和神经侵犯者的23.08%、24.14%、23.08%、29.17%、17.65%、20.00%,差异均有统计学意义(P<0.05);TNM分期中的Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期患者的CTCs计数分别为(1.24±0.14)个、(1.83±0.26)个、(15.30±1.87)个、(19.86±1.65)个,CTCs计数随着分期的升高而增加,差异有统计学意义(P<0.05)。结论膀胱癌患者CTCs表达与肿瘤分期关系密切,肿瘤分期越高,CTCs计数越高。
Objective To analyze the expression of circulating tumor cells(CTCs) in bladder cancer patients with different tumor stages(TNM).Methods Fifty-eight patients of bladder cancer in Tangdu Hospital,Air Force Military Medical University from June 2018 to September 2020 were selected as the research objects,and all patients were tested for CTCs.The positive conditions of CTCs in patients with different clinicopathological characteristics,including gender,pathological type,age,degree of differentiation,tumor size,total TNM stage,T stage,N stage,M stage,vascular invasion and nerve invasion,were compared.Results Among the 58 patients,22 were positive for CTCs,and there was no significant difference in CTCs positive rate between different gender,pathological type,age,differentiation and tumor size(P>0.05).The positive rates of CTCs in patients with total TNM stage(stage Ⅲ+Ⅳ),T_(3)+T_(4),N_(1)+N_(2)+N_(3),M_(1),vascular invasion and nerve invasion were 50.00%,51.72%,53.13%,80.00%,46.34%,and 47.37% respectively,which were significantly higher than 23.08%,24.14%,23.08%,29.17%,17.65%,and 20.00% in patients with stage Ⅰ+Ⅱ,T_(1)+T_(2),N_(0),M_(0),patients without vascular invasion,and patients without nerve invasion(P<0.05).The CTCs counts of TNM stage Ⅰ,Ⅱ,Ⅲ and Ⅳ were 1.24 ± 0.14,1.83 ± 0.26,15.30 ± 1.87,and 19.86 ± 1.65,respectively.The CTCs counts increased with the increase of TNM stage(P<0.05).Conclusion The expression of CTCs in bladder cancer is closely related to tumor stage.The higher the tumor stage,the higher the CTCs count.
