摘要
目的:探究及观察锁定加压钢板内固定对胫骨Pilon骨折患者踝关节功能及骨代谢状态的影响。方法:选取2019年1月-2020年6月本院收治的100例胫骨Pilon骨折患者为研究对象,将其根据随机数字表法分为解剖钢板内固定组(对照组)和锁定加压钢板内固定组(观察组),每组50例。比较两组骨折愈合时间、负重时间、治疗后不同时间Mazur评分结果、疼痛评分(VAS评分)及手术前后的骨代谢指标(血清ALP、BGP及OPG)。结果:观察组骨折愈合时间、负重时间均显著短于对照组,术后3、6、12个月Mazur评分均显著优于对照组,术后1、3、5 d的VAS评分均显著低于对照组,差异均有统计学意义(P<0.05)。两组术前骨代谢指标比较,差异均无统计学意义(P>0.05);观察组术后1、2、4周骨代谢指标均显著高于对照组,差异均有统计学意义(P<0.05)。结论:锁定加压钢板内固定对胫骨Pilon骨折患者踝关节功能及骨代谢状态的影响相对更好,因此在胫骨Pilon骨折患者中的应用价值较高。
Objective:To investigate and observe the influence of locking compression plate internal fixation for ankle joint function and bone metabolism state of patients with Pilon fracture of tibia.Method:A total of 100 patients with Pilon fracture of tibia treated in our hospital from January 2019 to June 2020 were chosen as the study objects,and they were divided into anatomical plate internal fixation group (the control group) and locking compression plate internal fixation group (the observation group) by the method of random number table,50 cases in each group.The fracture healing time,weight-bearing time,Mazur score and pain score(VAS score) at different time after treatment,bone metabolism indexes (serum ALP,BGP and OPG) before and after operation of two groups were compared.Result:The fracture healing time and weight-bearing time of the observation group were significant shorter than those of the control group,Mazur scores at 3,6 and 12 months after operation were significantly better than those of the control group,VAS scores at 1,3 and 5 d after operation were significantly lower than those of the control group,there were statistically significant differences (P<0.05).The bone metabolism indexes of two groups before operation compared,there were no statistically significant differences (P>0.05);the bone metabolism indexes of the observation group at 1,2 and 4 weeks after operation were all significantly higher than those of the control group,there were statistically significant differences (P<0.05).Conclusion:The influence of locking compression plate internal fixation for ankle joint function and bone metabolism of patients with Pilon fracture of tibia are relatively better,and its application value in patients with Pilon fracture of tibia is higher.
