Grassroots employment intention of the contract-based tuition-free medical students

摘要 目的调查订单定向医学生对政策相关内容的认知情况和就业意向,分析影响订单定向医学生基层就业意向的主要因素,为完善订单定向医学生政策提供建议。方法2020年6—7月,采用整群抽样方法,选取山东大学承担订单定向医学生培养任务的两所医学本科院校中的2018—2019级订单定向医学生作为研究对象,通过网络问卷调查订单定向医学生个人基本情况、对政策相关内容的认知情况、毕业后就业意向,并基于Logistic回归分析影响订单定向医学生基层就业意向的主要因素。结果回收有效问卷644份,有效应答率88.71%。参与调查的订单定向医学生对自己的责任和义务较为清楚,但对基层医疗卫生机构工作的了解程度偏低;约85%的学生表示毕业后愿意从事基层医疗卫生工作,但服务期满后愿意继续留在基层工作的学生不足10%;月均生活消费、报考时对自己的责任和义务了解程度、目前对基层医疗卫生机构工作了解程度是影响订单定向医学生基层就业意向的重要因素(P<0.05)。结论为实现政策设计初衷,使订单定向医学生能够真正在基层留得住、用得上,未来政府相关部门一方面应提高订单定向医学生对政策及基层的认识,提升订单定向医学生服务基层的荣誉感和责任感,另一方面还需着力改善基层医疗卫生工作环境,从根本上提升基层工作的吸引力。 Objective To investigate the policy-related cognition and employment intention of the contract-based tuition-free medical students and to analyze the major influencing factors of their grassroots employment intention,so as to provide reference for the improvement of relevant policies.Methods In this study,the 2018—2019 batches of contract-based tuition-free medical students from two medical universities in Jinan City selected by cluster sampling were surveyed by an online questionnaire from June to July 2020.The content of the questionnaire covered the respondents’basic information,policy-related cognition and employment intention.Logistic regression was used to analyze the major influencing factors.Results A total of 644 valid questionnaires were received,with an effective response rate of 88.71%.Most contract-based tuition-free medical students were clear about their responsibilities and obligations,but they had a low level of understanding of primary health institutions.Approximately 85%of the respondents said they were going to engage in primary health care after graduation.However,less than 10%of them showed willingness to stay at the primary health institutions after the expiration of their service.The main factors affecting grassroots employment intentions were average monthly living consumption,awareness of the responsibilities and obligations,and the current understanding of the healthcare work in primary health institutions(P<0.05).Conclusions In order to retain the contract-based tuition-free medical students in primary health institutions and give full play to their talents,we suggest raising the contract-based tuition-free medical students’cognition of policies and healthcare services in primary health institutions and their sense of honor and responsibility for serving the grassroots population,and on the other hand,improving the grassroots working environment so as to fundamentally increase the attractiveness of primary healthcare work.
