摘要
目的分析游离皮瓣移植联合骨段滑移术治疗胫骨骨髓炎骨缺损患者的临床效果。方法选取2015年10月至2020年11月中国人民解放军联勤保障部队第九八八医院胫骨骨髓炎骨缺损患者60例,按随机数字表法分为对照组与研究组,每组30例。对照组予以骨段滑移术治疗,研究组予以游离皮瓣移植联合骨段滑移术治疗。对比两组临床效果,并发症发生率,术前、术后1年下肢功能[美国特种外科医院(HSS)膝关节功能评分标准、踝关节功能Baird-Jackson评价系统]评分,术前、术后6 h视觉疼痛模拟评分法(VAS)评分。结果研究组优良率(93.10%,27/29)较对照组(68.97%,20/29)高,差异有统计学意义(χ^(2)=5.497,P<0.05)。术前,两组HSS、Baird-Jackson评分比较,差异未见统计学意义(P>0.05);术后1年,研究组HSS、Baird-Jackson评分较对照组高,差异有统计学意义(P<0.05)。术前,两组VAS评分比较差异未见统计学意义(P>0.05),术后6 h,研究组VAS评分较对照组低,差异有统计学意义(P<0.05)。研究组并发症发生率(3.33%,1/30)与对照组(6.67%,2/30)比较,差异未见统计学意义(P>0.05)。结论游离皮瓣移植联合骨段滑移术治疗胫骨骨髓炎骨缺损患者效果显著,可减轻疼痛,并发症发生率低,可提高预后效果,改善下肢功能。
Objective To analyze the clinical effect of free skin flap grafting combined with bone segment slippage on bone defect of tibial osteomyelitis.Methods Sixty patients with bone defect of tibial osteomyelitis in the First Division of Orthopedics Department of the 988 th Hospital of the Joint Logistic Support Force of the People’s Liberation Army from October 2015 to November 2020 were selected and divided into study group and control group according to random number table method,with30 cases in each group.The control group was treated with osteolisthesis.and the study group was treated with free skin flap transplantation combined with osteolisthesis.The clinical effects,incidence of complications,lower extremity function scores[HSS Knee-joint function scoring standard,Baird-Jackson ankle function evaluation system]and visual pain analogue scale(VAS)scores before and after surgery were compared between the two groups.Results The excellent and good rate of the study group(93.10%,27/29)was higher than that of the control group(68.97%,20/29),the difference was significant(χ2=5.497,P<0.05).Before operation,there was no significant difference in HSS or Baird Jackson scores between the two groups(P>0.05);One year after operation,the HSS and Baird Jackson scores of the study group were higher than those of the control group,and the differences were significant(P<0.05).Before operation,there was no significant difference in VAS score between the two groups(P>0.05).Six hours after operation,the VAS score of the study group was lower than that of the control group,and the difference was significant(P<0.05).There was no significant difference in the incidence of complications between the study group(3.33%,1/30)and the control group(6.67%,2/30)(P>0.05).Conclusions Free flap transplantation combined with bone segment slippage has significant effect on bone defect of tibial osteomyelitis,which can relieve pain,reduce complication rate,improve the prognosis and lower limb function.
