摘要
目的:探讨血栓弹力图评估前列腺增生经尿道前列腺电切术术后凝血状态的临床价值。方法:选择2019年12月-2020年12月南昌市第三医院收治的前列腺增生并施经尿道前列腺电切术的患者50例。比较手术前后凝血功能指标、血栓弹力图指标及术后凝血常规与血栓弹力图提示凝血功能异常比例,分析凝血常规与血栓弹力图诊断前列腺手术术后凝血功能异常的价值。结果:术后凝血功能指标中活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)和凝血酶时间(TT)均短于术前(P<0.05),纤维蛋白原(FIB)水平高于术前(P<0.05),术后血栓弹力图指标中凝血时间(R)、最大振幅(MA)和反应时间(K)小于术前(P<0.05),凝集块形成速度α(Angle-α)和凝血综合指数(CI)大于术前(P<0.05)。血栓弹力图提示高凝比例显著大于凝血常规(P<0.05),血栓弹力图灵敏度和特异度均高于凝血常规。结论:围术期血栓弹力图能有效评估经尿道前列腺电切术术后凝血功能,有利于早期发现高凝与低凝状态,为临床治疗提供指导。
Objective:To investigate the clinical value of thromboelastography in evaluating the coagulation status after transurethral resection of prostate for hyperplasia of prostate.Methods:From December 2019 to December 2020,50 patients with prostatoplasia who underwent transurethral resection of prostate were selected as the research subjects.Before and after surgery were performed conventional elastic graph examination,blood coagulation function and blood clots compared before and after surgery coagulation function index,thrombosis,elastic figure index,compared with conventional coagulation function and postoperative thrombosis elastic figure tips coagulant function abnormality ratio,elastic analysis of conventional coagulation function and thrombosis diagnosed prostate surgery the value of coagulant function abnormality.Results:The activated partial thromboplastin time(APTT),prothrombin time(PT)and thrombin time(TT)in the postoperative coagulation function indexes were shorter than those before the operation(P<0.05),fibrinogen(FIB)level was higher than before operation(P<0.05),and the clotting time(R),maximum amplitude(MA)and reaction time(K)in the postoperative thromboelastogram index were lower than before operation(P<0.05).The rate of agglutination formationα(Angle-α)and the coagulation index(CI)were higher than those before the operation(P<0.05).Thromboelastogram showed that the proportion of hypercoagulability was significantly higher than that of routine coagulation testers(P<0.05),and the sensitivity and specificity of thromboelastogram testers were higher than those of routine coagulation testers.Conclusion:Thromboelastography can effectively evaluate the coagulation status after transurethral resection of the prostate and provide guidance for clinical treatment.
