摘要
查阅近10年中西医防治恶性肿瘤靶向药物治疗后口腔黏膜炎的相关文献,对其中西医发病机制、防治方法展开系统整理与分析。本病的发病机制尚未明确,现代医学认为可能与靶向药物治疗的作用机制相关,防治措施以口腔护理、支持性治疗为主,皮质类固醇激素对依维莫司相关性口腔炎疗效显著。靶向药物治疗后口腔黏膜炎属中医口疮、口糜等范畴,以脏腑功能失调、火热上挠口舌为主要病机,病位以心、脾、肾为主,治则上以清热泻火、调理脏腑阴阳为主,可分为清热解毒、清心泻脾、清热祛湿、养阴清热、健脾泻火五种治法。扶正抗癌法联合靶向药物使用可预防口腔黏膜炎的发生。
This paper reviews the relevant literatures on the integrated Chinese and western medicine prevention and treatment of oral mucositis after targeted medicines of malignant tumors in recent 10 years,and systematically sorts out and analyzes the pathogenesis as well as prevention and treatment methods of integrated Chinese and western medicine in the treatment of the disease. The pathogenesis of the disease is not clear,and modern medicine considers that it may be related to the mechanism of targeted medicine therapy. The main prevention and treatment measures are oral care and supportive treatment,and corticosteroids has a significant curative effect on everolimus-related stomatitis. Oral mucositis after targeted medicines belongs to aphtha and oral erosion in Chinese medicine;the main pathogenesis is the dysfunction of viscera and bowels as well as fire-heat flaming upward to mouth and tongue;the main location of the disease lies in the heart,the spleen and the kidney. Treatments of the disease mainly focus on clearing heat and purging fire,as well as regulating yin and yang of viscera and bowels, which can be divided into five treatment methods, namely clearing heat and detoxifying,clearing the heart and purging the spleen,clearing heat and dispelling dampness,nourishing yin and clearing heat,as well as fortifying the spleen and purging fire. Targeted medicine combined with the method of reinforcing the healthy qi to fight against cancer can prevent the occurrence of oral mucositis.
