摘要
结节性红斑是一种以双下肢疼痛性红斑为主要临床表现的炎症性疾病,好发于小腿伸侧,多发生皮下脂肪小叶间隔病变,并伴有自身免疫性病变,其病程长、易复发。结节性红斑的病因病机较复杂,张鸣鹤教授结合多年临床实践经验认为,外感湿热、内生火热、内外合邪、热盛酿毒、热毒入血、炼液成痰、凝血成瘀、搏结腠理为其主要病机,血热有余、湿热不除为反复不愈的重要原因。热毒为其主要病理因素,当在热毒致病的基础上辨病施治,以清热凉血解毒、软坚活血散结为主线,自拟辨病主方如白花蛇舌草、半枝莲、连翘、丹皮、赤芍、生地榆、山慈菇、漏芦、桃仁、红花、莪术、荜澄茄,同时提出应根据患者体质及热毒痰瘀之轻重而辨证施治,其临床疗效显著。
Erythema nodosum is an inflammatory disease with painful erythema of lower extremities as the main clinical manifestation.It usually occurs on the extended side of the lower leg and often occurs with subcutaneous fat lobular septal disease.It is often accompanied by autoimmune lesions,with a long course and easy recurrence.Its etiology and pathogenesis are complicated.Professor ZHANG Ming-he combined with years of clinical practice experience owns an idea of the main pathogenes is that heat-toxicity entering blood makes liquid into phlegm and blocks blood and skin after having combined exogenous damp-heat and endogenous heat.The main reasons of recurrent and unhealed are excessive blood-heat and damp-heat.The main pathological factors are heat-toxicity.When the disease is diagnosed and treated on the basis of heat toxin,the main line is to clear heat,cool blood and detoxify,soften hard and promote blood circulation and dispel stagnation.Self-made main prescriptions for disease differentiation:Hedyotis Diffusa,Sculellaria Barbata,Forsythia,Peony Bark,Radix Paeoniae Rubra,Radix Rehmanniae Elm,Indian Iphigenia Bulb,Uniflower Swisscentaury Root,Peach Seed,Carthamus Tinctorius,Curcuma Zedoary,Piper Cubeba.At the same time,treat according to the patient's physique and the severity of heat toxin,phlegm and blood stasis.Professor Zhang's experience in treating erythema nodosa has significant clinical effects.
