气管内滴入布地奈德联合牛肺表面活性物质对预防极低出生体重早产儿慢性肺病的临床研究 被引量:3

摘要 目的探究气管内滴入布地奈德联合牛肺表面活性物质对预防极低出生体重早产儿慢性肺病(CLD)的临床效果。方法选取2019年6月至2021年1月在广东医科大学顺德妇女儿童医院新生儿科收治的早产儿60例作为观察对象,按照随机数字表法分为对照组与观察组,每组各30例。两组患儿均给予常规治疗,其中对照组患儿给予气管内滴注牛肺表面活性物质,观察组患儿给予气管内滴注牛肺表面活性物质和布地奈德的混合剂。比较观察组和对照组的撤机时间、机械辅助呼吸时间、CLD发生率、血气指标以及并发症发生率。结果观察组患儿CLD发生率明显低于对照组(P<0.05);观察组患儿的机械通气时间、撤机时间以及总住院时间均低于对照组(P<0.05);两组患儿治疗后动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))均得到明显改善,其中观察组患儿改善更为明显(P<0.05);两组患儿并发症发生率比较差异无统计学意义(P>0.05)。结论气管内滴入布地奈德联合牛肺表面活性物质可有效预防极低出生体重早产儿发生慢性肺病,明显改善患儿血气指标且不增加并发症发生风险。 Objective To investigate the clinical effect of intratracheal instillation of budesonide combined with bovine pulmonary surfactant on the prevention of chronic lung disease(CLD)in preterm infants with extremely low birth weight.Methods A total of 60 preterm infants admitted to the Department of Neonatology in Shunde Women and Children's Hospital of Guangdong Medical University from June 2019 to January 2021 were selected as observation objects and divided into 30 cases each in the control group and the observation group according to the random number table method.The infants in both groups were given conventional treatment.Besides,the control group was treated with intratracheal instillation of bovine pulmonary surfactant and the observation group was treated with intratracheal instillation of mixture of budesonide and bovine pulmonary surfactant.The withdrawal time,duration of mechanically assisted breathing,incidence of CLD,blood gas index and incidence of complications were compared between the observation group and the control group.Results The incidence of CLD in the infants of the observation group was significantly lower than that of the control group(P<0.05).The duration of mechanical ventilation,withdrawal time and total hospital stay in the infants of the observation group were lower than those of the control group(P<0.05).Arterial partial pressure of carbon dioxide(PaCO_(2))and oxygen tension(PaO_(2))in the infants of both groups improved significantly after treatment,and the improvement was more obvious in the infants of the observation group(P<0.05).The incidence of complications in the infants of the two groups was compared,and the difference was not statistically significant(P>0.05).Conclusion Intratracheal instillation of budesonide combined with bovine pulmonary surfactant can effectively prevent CLD in preterm infants with extremely low birth weight,and significantly improve the blood gas index of the infants without increasing the risk of complications.
