摘要
目的探讨埋线提升技术应用在面部年轻化后可能引起的并发症,并分析其原因,寻找改进措施。方法自2013年4月至2020年4月,中国医学科学院北京协和医学院整形外科医院瘢痕微创治疗中心共收治142例外院接受面部埋线提升术后出现并发症的患者。其中皮肤凹陷和面部轮廓不规则40例,颧弓区增宽合并面颊凹陷25例,术后感染20例,线材外露18例,线材可触及或可见12例,皮肤组织瘢痕增生或色素沉着12例,面神经损伤5例,鼻颊沟加深5例,术后血肿3例,术后过敏1例,感觉异常1例。积极给予对症治疗,包括手法复位,75%乙醇湿敷、压力绷带和静脉输注抗生素治疗、引流、取出部分线材、激光及激素治疗,并随访。结果其中于本院进行治疗和随访的139例患者的并发症均得到有效控制,其中131例患者自评治疗效果为满意,8例一般,无不满意者,修复者满意率为94.2%(131/139)。结论采用埋线提升技术行面部年轻化,一要在掌握面部解剖学基础上,选择正确的皮肤埋线层次,二要在术中操作时轻柔准确。两者的完美结合是取得面部年轻化提升术后良好效果的关键。
Objective To explore the potential complications associated with thread-lift sutures for facial rejuvenation,analyze the reasons and find improvement measures.Methods From April 2013 to April 2020,142 patients with complications after thread-lift sutures for facial rejuvenation were collected.There were 40 cases of skin depression and contour irregulation,25 cases of zygomatic arch widening and cheek depression,20 cases of postoperative infection,18 cases of threads exposure,12 cases of visible or palpable threads,12 cases of scar hyperplasia or pigmentation of skin,5 cases of facial nerve injury,5 cases of deep nasojugal groove,3 cases of postoperative hematoma,1 case of postoperative allergy and 1 case of paresthesia.Active symptomatic treatment was given to these patients,including manipulative reduction,75%alcohol wet dressing,pressure bandage,intravenous infusion of antibiotics,drainage,part threads removal,laser and hormone treatment.All patients were followed up after treatment.Results The complications of 139 patients were effectively controlled,of which 131 patients rated their treatment results as satisfaction,8 as general and 0 as unsatisfaction.The satisfaction rate of patients was 94.2%(131/139).Conclusion It is necessary to choose appropriate layer of skin to embed sutures on the basis of facial anatomy.Gentle and accurate operation is important.The combination of the above 2 points is the key to achieve good postoperative results.
