摘要
目的研究高龄患者与普通患者的结肠微小息肉的内镜下形态、发生部位等特点以及病理特征,来探讨高龄患者微小息肉切除的必要性。方法分析通过电子结肠镜确诊并行内镜下切除微小息肉(≤5 mm)患者的临床资料,按照年龄分为高龄组(≥75岁),普通组(<75岁),分别对2组患者在年龄、临床特点、内镜下表现特点、病理特征等资料进行对照分析。结果2组切除结肠息肉、结肠微小息肉人均数量比较,差异无统计学意义(P>0.05)。2组微小息肉内镜下大水、形态、部位比较,差异无统计学意义(P>0.05)。2组结肠微小息肉病理类型中腺瘤、伴低级别上皮内瘤变、增生性/炎性息肉比例比较,差异无统计学意义(P>0.05)。2组不同术式所卢比例比较,差异无统计学意义(P>0.05)。2组随访9~12个月,平均(10.3±1.6)个月,高龄组2例因不良事件死亡。结论高龄患者结肠微小息肉内镜及病理特点与普通组相比无明显差异,但仍存在微小息肉合并低级别上皮内瘤变的病例,因此在患者一般情况允许下仍建议进行内镜下息肉切除,但需要考虑患者身体的综合情况。
Objective To investigate the endoscopic morphous,location and pathological features of colon micropolyps in elderly patients,and to explore the necessity of resection of colon micropolyps in elderly patients.Methods The clinical data of patients with colon micropolyps who were diagnosed by electronic colonoscopy and underwent endoscopic resection of tiny polyps(≤5mm)were analyzed.According to patient’s age,the patients were divided into the senior group(≥75 years)and control group(<75 years),and the data including age,clinical characteristics,endoscopic performance characteristics and pathological characteristics were observed and compared between the two groups.Results There were no significant differences in the number of patients with micropolypectomies,morphous and position of colon micropolyps.and the proportion of adenomas,low-grade intraepithelial neoplasia and hyperplastic/inflammatory polyps between the two groups(P>0.05).There was no significant difference in the proportion of different surgical ways between the two groups(P>0.05).The patients in both groups were followed up for 9~12 months,with an average duration being 10.3±1.6 months,and 2 patients in senior group died of adverse events.Conclusion There are no significant differences in colon micropolyp endoscopic and pathological features between the elderly patients and the younger people,but there are still cases of micropolyp complicated by low-grade intraepithelial neoplasia.Therefore,endoscopic polypectomy is recommended when the general condition of the patients is better,but the comprehensive condition of the patients should be considered.
