额叶背外侧癫癎患者认知损害影响因素及其脑电图异常的相关性分析 被引量:1

Cognitive Impairment Influencing Factors in Patients with Dorsolateral Frontal Lobe Epilepsy and the Correlation Analysis between Cognitive Impairment and EEG Abnormalities

摘要 目的探究额叶背外侧癫癎患者认知损害的影响因素及认知损害与脑电图异常的相关性。方法选择2018年4月至2020年4月收治的82例额叶背外侧癫癎患者为研究对象,根据认知功能测试结果,将患者分为正常组(47例)和认知损害组(35例)。比较两组患者的一般临床资料、认知测试水平和脑电图检查结果;使用Logisitic回归分析患者认知损害的影响因素,构建列线图预测模型并评价其准确度;分析脑电图异常与患者认知功能的相关性。结果82例患者发病年龄≥20岁、受教育年限≤12年、病程≥8年、发作频率每月≥4次、发作持续时间≥1.5 min、抗癫癎药(单药或多药联用)均是影响认知功能的危险因素;列线图模型预测的准确度较高;认知损害组患者的顺背测试水平与正常组比较,差异无统计学意义(P>0.05);认知损害组其他认知测试指标及脑电图异常病例数均明显高于正常组(P<0.05);脑电图异常与认知损害呈明显负相关。结论脑电图异常与额叶背外侧癫癎患者认知损害密切相关,认知损害的预测模型构建可为患者的风险因素识别提供参考价值。 Aim To explore the influencing factors of cognitive impairment in patients with dorsolateral frontal lobe epilepsy and the correlation between cognitive impairment and abnormal electroencephalogram(EEG).Methods Eighty-two patients with dorsolateral frontal lobe epilepsy treated in our hospital from April 2018 to April 2020 were selected as subjects.According to the results of cognitive function test,the patients were divided into a normal group(47 cases)and a damage group(35 cases).The general data,cognitive function test level and EEG results of the two groups were compared.The influencing factors of cognitive function were analyzed by Logisitic regression.The prediction model of the linear graph was constructed and its accuracy was evaluated.The correlation between abnormal EEG and cognitive function was analyzed.Results Age of onset more than 20 years,years of education more than 12 years,course of illness more than 8 years,frequency of attacks more than 4 times/month,duration of attacks more than 1.5 min,and antiepileptic drugs(type)are all risk factors affecting the cognitive function of patients.The prediction accuracy of the linear graph model is higher.there is no statistically significant difference in the level of traverse and back test between patients in the injury group and the normal group(P>0.05).Other cognitive function test indicators and the number of abnormal EEGs are significantly higher than those in the normal group(P<0.05).There is a significant negative correlation between EEG abnormalities and cognitive impairment.Conclusion Abnormal EEG is closely related to patients’cognitive function.The construction of a predictive model for predicting cognitive dysfunction in patients with dorsolateral frontal lobe epilepsy provides reference value for the identification of clinical risk factors.
