颅内动脉瘤患者应用早期高压氧综合疗法对脑功能和神经功能的影响 被引量:4

EFFECTS OF EARLY HYPERBARIC OXYGEN COMBINATION THERAPY ON BRAIN FUNCTION AND NERVE FUNCTION IN PATIENTS WITH INTRACRANIAL ANEURYSMS

摘要 目的观察颅内动脉瘤患者应用早期高压氧综合疗法对脑功能和神经功能的影响。方法选取2016年1月—2019年1月该院神经外科接收的颅内动脉瘤患者76例,采用随机数字表法,将上述所有患者分为观察组与对照组各38例。二组患者均采取了颅内动脉瘤夹闭术,术后,对照组采用常规治疗,观察组在对照组基础上采取早期高压氧(HBO)综合治疗,对比二组疗效。结果二组治疗前后的组内、组间神经功能缺损程度(CNDF)和日常生活活动(ADL)评分、大脑中动脉流速及症状性脑血管痉挛(CVS)情况比较差异均有统计学意义(均P<0.05)。不同时期HBO的治疗效果组间比较差异均有统计学意义(均P<0.05)。结论早期高压氧综合疗法可有效改善患者神经功能缺损程度,促进大脑中动脉流速增加,较好改善脑血管痉挛情况,有效促进脑功能和神经功能的恢复。 Objective To observe the effect of early hyperbaric oxygen combined therapy on brain function and nerve function in patients with intracranial aneurysms.Methods Seventy six patients with intracranial aneurysm admitted to the neurosurgery department of our hospital from January 2016 to January 2019 were selected.All the above patients were divided into observation group and control group by random number table method, 38 patients in each group.Patients in both groups underwent intracranial aneurysm cliplication.After surgery, routine treatment was used in the control group, and combined treatment with early hyperbaric oxygen(HBO) was adopted in the observation group on the basis of the control group, and the efficacy of the two groups was compared.Results There were significant differences in the degree of neurological impairment(CNDF),Activity of Daily Living(ADL) score, middle cerebral artery flow rate and symptomatic cerebral vasospasm(CVS) before and after treatment between the two groups(P< 0.05).There were differences in HBO treatment effect between groups at different periods(P< 0.05).Conclusion Early hyperbaric oxygen therapy can effectively improve the degree of nerve function defect, promote the increase of middle cerebral artery flow rate, improve cerebral vasospasm, and effectively promote the recovery of brain function and nerve function.
