摘要
目的探究T-scanⅢ对咬[牙合]力的控制以及测量牙槽骨高度、龈沟液中OPG和RANKL水平变化对牙周炎患者固定义齿修复效果的评价价值。方法选取2018年1月至2019年6月于本院进行固定义齿修复的20例牙周炎伴前牙缺失患者,其患牙基牙牙槽骨吸收在1/2~2/3。为患者采取固定义齿修复,于戴牙时借助T-scanⅢ系统调颌。在修复前、修复后借助该系统对牙槽骨高度、龈沟液中OPG和RANKL水平及咬[牙合]力心点位置进行检测。结果处于正中咬位置时,咬力心点距X轴垂直距离,修复前长于修复后,差异均存在统计学意义(P<0.05)。修复后龈沟液中OPG浓度高于修复前,修复后RANKL浓度低于修复前,修复后RANKL/OPG比值低于修复前,差异均存在统计学意义(P<0.05)。此外,修复前、修复后基牙牙槽骨高度未发生明显变化,差异无统计学意义(P>0.05)。结论对于牙槽骨吸收在1/2~2/3的患牙,可在牙周治疗及去髓手术后采取固定义齿修复,并在修复前中后以T-scanⅢ系统检测,可使患者得到有效的治疗。
Objective To investigate the value of T-scanⅢin the control of bite force and the measurement of alveolar bone height and OPG and RANKL levels in gingival crevicular fluid for the evaluation of fixed denture in patients with periodontitis.Methods Twenty patients with periodontitis and anterior teeth loss who underwent fixed dentures in our hospital from January 2018 to June 2019 were enrolled in the alveolar bone between 1/2-2/3.A fixed denture is used for the patient to adjust the jaw with the T-scanⅢsystem when wearing the tooth.The system was used to detect the alveolar bone height,the OPG and RANKL levels in the gingival crevicular fluid,and the position of the occlusal heart point before and after repair.Results At the mid-occlusion position,the occlusal force point was perpendicular to the X-axis.Before the repair,the difference was statistically significant(P<0.05).The concentration of OPG in the gingival crevicular fluid after repair was higher than that before repair.The RANKL concentration after repair was lower than that before repair.The RANKL/OPG ratio was lower than that before repair,and the difference was statistically significant(P<0.05).In addition,there was no significant change in the alveolar bone height of the abutment before and after repair,and the difference was not statistically significant(P>0.05).Conclusion For the teeth with alveolar bone resorption between 1/2-2/3,fixed denture can be repaired after periodontal treatment and demyelination,and detected by T-scanⅢsystem before and after repair.In order to get effective treatment for patients.
