Rh分型同型输注对于反复输血患者的必要性分析 被引量:18

Analysis of the necessity of Rh phenotype-matched transfusion for patients with repeated blood transfusion

摘要 目的:统计Rh血型系统D、C、c、E、e表型在献血者和郑州大学第一附属医院拟输血患者中的分布情况,分析临床常规检测Rh表型及其同型输注的必要性,为临床输血安全性提供依据。方法:采用血清学方法对RhD阳性的献血者和拟输血患者样本进行Rh表型检测和分析,抗人球蛋白法进行意外抗体筛查及鉴定,并分析Rh系统抗体产生的影响因素。结果:在患者本次入院输血前检出的379例意外抗体中,Rh血型系统198例,占52.24%。RhD阳性献血者和拟输血患者分型均主要以CCee和CcEe为主,其次为Ccee、ccEE和ccEe。选择Rh分型同型输血产生意外抗体的概率降低(P<0.001),并且非同型输血中输血次数是Rh系统抗体产生的影响因素(P<0.001)。结论:实施Rh分型同型输血可在一定程度上防止或减少高频抗体的产生,尤其是有利于降低反复输血患者输血后产生Rh系统同种抗体的概率。 Aim:To analyze the distribution of Rh blood group system phenotypes D,C,c,E,e among blood donors and patients who need blood transfusion in the First Affiliated Hospital of Zhengzhou University,and to assess the necessity of routine clinical test of Rh phenotype and homotypic transfusion,which is of vital importance for the safety of clinical blood transfusion.Methods:Serological methods were used to perform Rh phenotype detection and analysis on samples from RhD-positive blood donors and patients undergoing blood transfusion,anti-human globulin method was used to detect unexpected antibody screening and identification,and the influencing factors of Rh system antibodies were analyzed.Results:Among the 379 cases that were positive for unexpected antibodies,198 cases(52.24%)were Rh blood group system.CCee and CcEe were dominant in RhD positive blood donors and patients,followed by Ccee,ccEE and ccEe.The detection rate of unexpected antibodies in Rh phenotype-matched transfusion group was reduced(P<0.001),and the number of blood transfusion in non-isotype transfusion was the influencing factor of Rh system antibody production(P<0.001).Conclusion:Rh phenotype-matched transfusion might prevent or reduce the production of high-frequency antibodies,which is especially beneficial to reduce the incidence of Rh system alloantibodies after transfusion in patients with repeated blood transfusion.
