摘要
目的探讨成人咽喉反流性疾病(laryngopharyngeal reflux disease,LPRD)发病影响因素及与睡眠状态的关系。方法纳入达州市中心医院耳鼻咽喉科就诊患者共454例临床资料,其中确诊LPRD共83例设为A组,非LPRD共371例设为B组;比较两组基线临床特征资料水平,采用多因素逐步多元Logistic回归分析法评估成人LPRD发病独立影响因素,分析LPRD与睡眠状态相关性。结果①A组匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)评分、医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HADS)-焦虑评分及HADS-抑郁评分均显著高于B组(P<0.05);②伴睡眠障碍LPRD患者HADS-焦虑评分和HADS-抑郁评分异常比例均显著高于睡眠正常者(P<0.05);③多因素分析结果显示,睡眠状态是成人LPRD发病独立影响因素(P<0.05)。结论睡眠状态与成人LPRD发病独立相关,同时睡眠障碍还可能导致患者负面情绪出现或加重。
OBJECTIVE To investigate the influencing factors of adult patients with LPRD and its correlation with sleep state.METHODS Clinical data of 454 patients in Department of ENT were retrospectively chosen in the period from January 2017 to June 2019 in our hospital.The 83 patients with LPRD and 371 patients without LPRD were separately set as group A and group B.The clinical characteristics of the two groups were compared,the independent influencing factors of adult patients with LPRD were evaluated by stepwise multiple logistic regression analysis,and the correlation between LPRD and sleep status was analyzed.RESULTS 1.The PSQI scores,HADS anxiety scores and HADS depression scores in group A were significantly higher than those in group B(P<0.05).2.The abnormal rate of HADS anxiety score and HADS depression score in LPRD patients with sleep disorders were significantly higher than those in patients with normal sleep(P<0.05).3.The results of multivariate analysis showed that sleep state was the independent factor of adult patients with LPRD(P<0.05).CONCLUSION Sleep state is independently related to the onset of adult patients with LPRD,and sleep disorders may also lead to the occurrence or aggravation of negative emotions in patients.
