摘要
目的探究股骨近端防旋髓内钉(PFNA)内固定联合自制十七味大活血胶囊治疗股骨粗隆间骨折临床效果。方法选择该院收治的80例股骨粗隆间骨折患者,随机分组为对照组和研究组各40例,对照组给予PFNA内固定治疗,研究组给予PFNA内固定治疗联合十七味大活血胶囊治疗,比较两组疗效、髋关节功能、X线片断端骨痂生长评分、并发症发生率。结果经治疗,研究组治疗优良率为92.50%(32/40),显著高于对照组的75.00%(30/40)(P<0.05);治疗12周内,两组患者X线片断端骨痂生长评分呈增长趋势,组内差异有统计学意义(P<0.05),且研究组患者治疗后2周、4周、8周、12周的X线片断端骨痂生长评分均显著高于对照组(P<0.05);术后6个月,两组患者Harris髋关节功能中疼痛、关节功能、关节活动度及畸形评分较治疗前均显著提高(P<0.05),且研究组高于对照组(P<0.05);研究组并发症发生率显著低于对照组(P<0.05)。结论 PFNA内固定联合自制十七味大活血胶囊治疗股骨粗隆间骨折疗效显著,在促进骨痂形成方面具有明显优势,有利于促进患者髋功能恢复、减少术后并发症。
Objective To explore the clinical effects of proximal femoral nail anti-rotation(PFNA) internal fixation combined with Self-made Shiqiwei Dahuoxue Capsule(自制十七味大活血胶囊) in the treatment of femoral intertrochanteric fractures.Methods Eighty patients with femoral intertrochanteric fractures treated in our hospital were selected and randomly divided into control group and study group,with 40 cases in each group.The control group was treated with PFNA internal fixation,and the study group was given PFNA internal fixation combined with Shiqiwei Dahuoxue Capsule.The efficacy,hip joint function,X-ray bone stump callus growth score and incidence of complications were compared between the two groups.Results After treatment,the excellent and good rate in the study group was significantly higher than that in the control group[92.50%(37/40) vs 75.00%(30/40)](P<0.05).Within 12 weeks of treatment,the X-ray bone stump callus growth scores of the two groups showed an increasing trend(P<0.05),and the X-ray bone stump callus growth scores of the study group at 2,4,8 and 12 weeks after treatment were significantly higher than those of the control group(P<0.05).Six months after surgery,the scores of Harris hip joint function of pain,joint function,joint mobility and deformity in the two groups were significantly improved compared with those before treatment(P<0.05),and the scores in the study group were higher than those in the control group(P<0.05).The incidence rates of complications in the study group were significantly lower than those in the control group(P<0.05).Conclusion PFNA internal fixation combined with Self-made Shiqiwei Dahuoxue Capsule has significant efficacy on treating femoral intertrochanteric fractures,and has obvious advantages on promoting callus formation,and it is conducive to promoting hip joint function recovery and reducing postoperative complications.
