摘要
目的分析尿液培养阳性标本中病原菌的分布情况及两种常见肠球菌的耐药性,为指导临床合理使用抗菌药物提供依据。方法回顾实验室2019年8-9月送的1102例检尿液培养标本,统计分析其阳性标本的病原菌类型及药敏情况。结果经筛选共有887例纳入统计范围。经统计,革兰阴性杆菌685例(77.23%),其中大肠埃希菌共有356例(40.14%)。革兰阳性球菌106例(11.95%),其中屎肠球菌52例(5.86%),粪肠球菌32例(3.61%)。真菌96例(10.82%)。屎肠球菌对左氧氟沙星,氨苄西林,青霉素,红霉素的耐药率超过90.00%,粪肠球菌对四环素的耐药率超过90.00%,两种肠球菌对部分药物耐药性差异较大,但对万古霉素等药物的耐药性依旧较低。结论尿液阳性培养结果的病原菌分布仍以大肠埃希菌占比最高,但肠球菌引起的尿路感染也应同样不容忽视,细菌感染的统计数据对临床合理使用抗菌药物有重要意义。
Objective To understand and analyze the distribution of various pathogenic bacteria in urine positive specimens and the drug resistance of two common Gram-positive cocci,so as to provide evidence for rational use of antibiotics in clinic.Methods One thousand one hundred and two urine culture samples from August to September in 2019 were reviewed,and the pathogen types and drug sensitivity of positive samples were analyzed.Results After screening,a total of 887 cases were included in the statistical range.According to statistics,there were 685 cases of gram-negative bacilli(77.23%),including 356 cases of Escherichia coli(40.14%).There were 106 cases of gram positive cocci(11.95%),including 52 cases of Enterococcus faecium(5.86%)and 32 cases of Enterococcus faecalis(3.61%).Fungi were found in 96 cases(10.82%).The resistance rate of Enterococcus faecium to levofloxacin,ampicillin,penicillin and erythromycin was more than 90.00%,and the resistance rate of Enterococcus faecalis to tetracycline was more than 90.00%.There was a great difference between the two kinds of Enterococcus,but the resistance to vancomycin was still low.Conclusion Escherichia coli accounted for the highest proportion of pathogenic bacteria in urine positive culture results.However,the urinary tract infection caused by Enterococcus should not be ignored.The statistical data of bacterial infection is of great significance for the rational use of antibiotics in clinic.
