摘要
目的分析关节镜下微创钢板固定手术治疗对促进关节术后愈合、关节功能恢复的效果。方法收集2018年6月~2020年6月我院收治的60例胫胫骨平台骨折(TPF)患者作为研究对象,按手术方式不同分为观察组(29例)和对照组(31例),对照组采用切开复位钢板内固定术治疗,观察组采用关节镜下微创锁定钢板固定术(MIPPO)治疗,比较两组术后愈合、膝关节功能恢复情况,术后不同时间段疼痛程度,随访6个月观察患者生存质量。结果观察组骨折愈合时间短于对照组,差异有统计学意义(P<0.05);术后1、2、3、7天,观察组VAS均低于对照组,差异有统计学意义(P<0.05);观察组Lysholm评分高于对照组,且膝关节功能优良率高于对照组,差异有统计学意义(P<0.05);观察组屈膝最大、最小角度均优于对照组,SF-36各项评分均高于对照组,差异有统计学意义(P<0.05)。结论与传统切开复位内固定术相比,关节镜下微创钢板固定手术治疗胫骨平台骨折有助于患者术后骨折愈合,改善膝关节功能,提高其生活质量,可作为治疗胫骨平台骨折的首选方案。
Objective To analyze the effect of arthroscopic minimally invasive plate fixation surgery on promoting joint healing and joint function recovery.Methods 60 cases of tibial plateau fracture(TPF)patients admitted to our hospital from June 2018 to June 2020 were collected as the research objects.According to different surgical methods,they were divided into observation group(29 cases)and control group(31 cases).The control group was treated with open reduction and internal fixation with steel plate.The observation group was treated with arthroscopic minimally invasive locking plate fixation(MIPPO).The postoperative healing and knee function recovery of the two groups were compared,and the degree of pain at different time periods after surgery was compared.Follow up for 6 months to observe the patient's quality of life.Results The fracture healing time of the observation group was shorter than that of the control group,the difference was statistically significant(P<0.05);At 1,2,3,and 7 d after operation,the VAS of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05);The Lysholm score of the observation group was higher than that of the control group,and the excellent and good rate of knee joint function was higher than that of the control group,the difference was statistically significant(P<0.05);The observation group's maximum and minimum knee flexion angles were better than those of the control group,and the scores of SF-36 were higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion Compared with traditional open reduction and internal fixation,arthroscopic minimally invasive plate fixation for tibial plateau fractures can help patients with postoperative fracture healing.Improving the function of the knee joint and improving its quality of life can be used as the first choice for the treatment of tibial plateau fractures.
