256层螺旋CT血管造影不同对比剂用量在胸痛患者诊断中的价值 被引量:3

摘要 目的探讨256层螺旋CT血管造影不同对比剂用量在胸痛患者诊断中的价值。方法选取于我院进行CT血管造影检查的胸痛患者1317例,随机将其分为A组(557例,4.0 mL/s注射0.7 mL/kg对比剂)和B组(760例,4.5 mL/s注射1.0 mg/kg对比剂)。比较两组不同血管断面CT值、图像噪声对比度、信噪比、造影图像质量评分及不良反应发生情况。结果B组的肺动脉、冠状动脉、降主动脉CT值略高于A组(P>0.05)。B组的肺动脉、冠状动脉、降主动脉图像噪声对比度和信噪比略高于A组(P>0.05)。B组的造影图像质量评分略高于A组(P>0.05)。A组的不良反应总发生率显著低于B组(P<0.05)。结论应用256层螺旋CT血管造影诊断胸痛时,以较低速度注射较低用量的对比剂,患者不同血管断面显示良好,造影图像质量也较好,能够满足临床诊断需求,且能降低不良反应发生率。 Objective To explore the diagnostic value of 256 slice spiral CT angiography with different dosage of contrast agent in patients with chest pain.Methods A total of 1317 chest pain patients who underwent CT angiography in our hospital were selected and randomly divided into group A(557 cases,4.0 mL/s injection of 0.7 mL/kg contrast agent)and group B(760 cases,4.5 mL/s injection of 1.0 mg/kg contrast agent).The CT values,image noise contrast and signal-to-noise ratio of different blood vessel sections,contrast image quality score and occurrence of adverse reactions were compared between the two groups.Results The CT values of pulmonary artery,coronary artery and descending aorta in the group B were slightly higher than those in the group A(P>0.05).The image noise contrast and signal-to-noise ratio of the pulmonary artery,coronary artery and descending aorta in the group B were slightly higher than those in the group A(P>0.05).The contrast image quality score in the group B was slightly higher than that in the group A(P>0.05).The total incidence of adverse reactions in the group A was significantly lower than in the group B(P<0.05).Conclusion The contrast agent is injected at a lower speed with a lower dosage when 256 slice spiral CT angiography is used to diagnose chest pain,then the different vascular sections of the patients can be well displayed,and the contrast image quality is also good.This dosage can meet the needs of clinical diagnosis and reduce the incidence of adverse reactions.
