摘要
目的:探讨三拗汤联合止嗽散加减在风寒袭肺型急性支气管中的治疗效果及对肺功能指标的影响。方法:选择116例风寒袭肺型急性支气管炎患者,按照自愿原则将患者分为观察组与对照组,每组58例。对照组患者采用常规西药治疗(如抗生素、镇咳、平喘、祛痰),观察组患者采用三拗汤与止嗽散对症加减而成的中药汤剂内服治疗,7 d为一个疗程,连续服用4个疗程,观察两组患者治疗效果、中医证候积分变化情况以及患者肺功能指标和生活质量指标改善情况。结果:①治疗后观察组的总有效率达到了93.10%(54/58),对照组总有效率为74.14%(43/58),差异有统计学意义(P<0.05);②治疗前两组患者中医证候(咳嗽、痰多、气喘、干湿性啰音、恶寒)积分的差异无统计学意义(P>0.05),治疗后两组患者的中医证候积分均降低,观察组患者积分降低程度更为明显,比较差异有统计学意义(P<0.05);③治疗前肺功能指标[肺活量(Vital Capacity,VC)、1 s用力呼气肺容积(Forced Expiratory Volume in 1 second,FEV_(1))、用力肺活量(Forced Vital Capacity,FVC)、最大通气量(Maximum Voluntary Ventilation,MVV)]均无差异性(P>0.05),治疗后观察组患者肺功能改善情况优于对照组(P<0.05);④治疗后观察组退热时间、喘憋时间以及肺部啰音消失时间均比对照组短,差异有统计学意义(P<0.05);⑤治疗后观察组患者的生活质量指标(健康状况、精神健康、生理机能、社会功能)评分均高于对照组患者,比较差异有统计学意义(P<0.05)。结论:三拗汤与止嗽散加减配合用于风寒袭肺证型急性支气管炎患者的治疗中,与常规西药治疗比较,在改善肺功能及生活质量方面具有明显的效果,同时治疗后的患者有效比例与对照组比较也更高,因此,中药汤剂合用在治疗上述疾病方面比单纯西药治疗更为有效,值得在治疗中优选。
Objective:To investigate efficacy of the San'ao decoction(三拗汤)plus Zhisou San(止嗽散)on acute bronchitis of the Fenghan Xifei syndrome(风寒袭肺证).Methods:116 patients were divided into the observation group and control group,58 cases in each group.The control group was treated with conventional western medicine(such as antibiotics,antitussive,antiasthmatic,expectorant).The observation group was treated with the San’ao decoction plus Zhisou San,7 days as a course,for 4 courses.Results:①The total efficiency was 93.10%(43/58)in the observation group,and 74.14%(43/58)in the control group(P<0.05).②TCM syndrome(cough,phlegm,asthma,dry and wet rales,cold)scores in two groups after the treatment were decreased,and the degree of scores reduction in the observation group was more obvious(P<0.05).③The pulmonary function indexes in the observation group after the treatment were significantly better than the control group(P<0.05).④After treatment,the time of antipyretic,asthmatic holding time and disappearance time of lung rhonchus in the observation group were shorter than the control group(P<0.05).⑤After treatment,the scores of life quality indexes in the observation group were higher than the control group(P<0.05).Conclusion:The San′ao decoction plus Zhisou San was more effective on acute bronchitis of the Fenghan Xifei syndrome,and is worthy of promotion.
