摘要
目的探讨循环肿瘤细胞(CTCs)和CTCs的HIF-1a表达与晚期乳腺癌病理特征及预后的相关性。方法选取2014年12月至2017年12月间商丘市第一人民医院收治的200例晚期乳腺癌患者,对不同病理特征与生存情况患者的CTCs和CTCs的HIF-1a表达水平进行比较,分析CTCs和CTCs的HIF-1a表达与晚期乳腺癌病理特征及预后的相关性。结果患者CTCs阳性125例,肿瘤直径>1.5cm、存在脉管侵犯、TNM分期为Ⅳ期、存在淋巴转移和人表皮生长因子受体2(HER-2)阳性患者的CTCs表达阳性率均较高,差异均有统计学意义(均P<0.05)。CTCs阳性患者的总生存时间(OS)和无进展生存时间(PFS)均低于CTCs阴性患者,差异均有统计学意义(均P<0.05)。125例CTCs阳性细胞中缺氧诱导因子-1α(HIF-1α)阳性101例,肿瘤直径>1.5cm、存在脉管侵犯、TNM分期为Ⅳ期、存在淋巴转移和HER-2阳性患者的CTCs的HIF-1α阳性表达率较高,差异均有统计学意义(均P<0.05)。CTCs的HIF-1α阳性患者的OS和PFS均低于CTCs的HIF-1α阴性患者,差异均有统计学意义(均P<0.05)。患者的肿瘤直径、TNM分期、淋巴结转移、脉管转移、HER-2、OS和PFS与CTCs及CTCs的HIF-1α表达均呈正相关,差异均有统计学意义(均P<0.05)。结论CTCs和CTCs的HIF-1a表达与晚期乳腺癌患者病理特征及预后均呈相关性,可作为治疗效果实时监测的重要依据。
Objective To study the association between circulating tumor cells(CTCs)and the expression of hypoxia-inducible factor-1α(HIF-1 a)in CTCs and the pathological characteristics and prognosis of advanced breast cancer.Methods In this study,200 patients with advanced breast cancer treated at Shangqiu First People’s Hospital were selected from December 2014 to December 2017.The expression of CTCs and HIF-1 a in CTCs were compared among patients of different pathological characteristics and survival conditions.The association between the expression of CTCs and HIF-1 a in CTCs and the pathological characteristics and prognosis of advanced breast cancer was analyzed.Results CTCs-positive was discovered in 125 patients.The positive rate of CTCs was high in patients with tumor diameter more than 1.5 cm,vascular metastasis,TNM stage IV,lymph node metastasis and human epidermal growth factor receptor 2(HER2)-positive(all P<0.05).Overall survival(OS)and progression-free survival(PFS)were lower in CTCs-positive patients than in CTCs-negative patients(all P<0.05).HIF-1α-positive was reported in101 CTCs-positive patients.The positive rate of HIF-1 a in CTCs was high in patients with tumor diameter more than 1.5 cm,vascular invasion,TNM stage IV,lymph node metastasis and HER-2-positive(all P<0.05).OS and PFS were lower in HIF-1α-positive patients lower than HIF-1α-negative patients(all P<0.05).Tumor diameter,TNM stage,lymph node metastasis,vascular metastasis,HER-2,OS and PFS were positively associated with expression of CTCs and HIF-1αin CTCs.Conclusion The expression of CTCs and HIF-1 a in CTCs is positively associated with the pathological characteristics and prognosis of advanced breast cancer,which can be used as an important basis for real-time monitoring of treatment effect in the future.
