摘要
目的:观察关节腔内注射复方倍他米松注射液联合体外冲击波治疗疼痛性肩关节挛缩症的疗效。方法:选取疼痛性肩关节挛缩症患者70例,随机分为对照组(体外冲击波治疗)和观察组(关节腔内注射复方倍他米松注射液联合体外冲击波治疗),每组35例。治疗后两组患者均进行同样康复锻炼,主要观察指标为治疗前、治疗1个疗程以及3个月后受累肩部的活动范围(ROM),次级指标为两组患者肩关节自评量表得分、治愈情况以及肌力评分。结果:两组患者治疗1个疗程后的总有效率比较无统计学差异(P>0.05),但治疗3个月后的总有效率观察组高于对照组(P<0.05)。两组患者接受治疗后肩关节ROM均有改善,1个疗程后的近期疗效和3个月后的远期疗效均显示观察组肩关节ROM优于对照组(均P<0.05)。两组患者接受治疗后肩关节自评量表评分较治疗前均有改善,且观察组治疗1个疗程后的评分和治疗3个月后的评分均优于对照组(均P<0.05)。两组患者治疗后肌力评分均有改善(均P<0.05),但两组患者同时间段肌力评分比较均无统计学差异(均P>0.05)。结论:相比于单纯的体外冲击波治疗,关节腔内注射复方倍他米松注射液联合体外冲击波治疗疼痛性肩关节挛缩症效果更为显著,能有效缓解肩关节疼痛情况,增加肩关节活动范围,提高患者生活质量。
Objective:To observe the effect of intra-articular injection of compound betamethasone injection combined with extracorporeal shock wave in the treatment of painful shoulder joint contracture.Methods:70 patients with painful shoulder joint contracture were randomly divided into control group(extracorporeal shock wave therapy)and observation group(intra-articular injection of compound betamethasone injection combined with extracorporeal shock wave therapy),35 cases each.After treatment,both groups of patients received the same rehabilitation exercise.The main observation indicators were ROM of the affected shoulder before treatment,1 course of treatment and 3 months later.The secondary indexes were the scores of shoulder joint self-rating scale,cure and muscle strength.Results:The total effective rate of the two groups after 1 course of treatment was not statistically different(P>0.05),but the total effective rate of the observation group after 3 months of treatment was higher than that of the control group(P<0.05).The short-term curative effect after 1 course of treatment and the long-term curative effect after 3 months showed that the ROM of shoulder joint in observation group was better than those in control group(all P<0.05).After treatment,the scores of shoulder joint self-rating scale of two groups were improved in comparison with those before treatment,and the scores of observation group after 1 course of treatment and the scores after 3 months of treatment were better than those of control group(all P<0.05).The muscle strength scores of two groups improved after treatment(all P<0.05),but there was no statistical difference in muscle strength scores of two groups at the same time period(all P>0.05).Conclusion:Compared with simple extracorporeal shock wave therapy,intra-articular injection of compound betamethasone injection combined with extracorporeal shock wave is more effective in treating painful shoulder joint contracture,which can effectively relieve shoulder joint pain,increase the ROM,and improve the life quality of patients.
