摘要
目的分析门静脉血栓(PVT)患者的临床特点,为早期预防和治疗PVT提供临床依据。方法回顾性分析73例PVT患者的一般资料、实验室及影像学检查结果、治疗及预后情况。结果73例PVT患者中,有内镜治疗史39例,脾切除史10例;肝功能Child分级中,A级33例,B级31例,C级9例;病因分布中,肝硬化67例,肝癌5例,胰腺炎1例;平均D-二聚体水平(2.63±0.61)μg/mL,平均门静脉主干(MPV)内径(14.05±2.23)mm,平均门静脉血流速度(14.64±2.72)cm/s;血栓位于MPV 48例;采用抗凝治疗59例,溶栓治疗4例,介入治疗5例,外科手术治疗3例。结论乙、丙型病毒性肝硬化、肝功能Child分级在B级以上、既往有内镜治疗及脾切除史、MPV内径增宽、门静脉血流速度降低可能是PVT的危险因素,及时进行早期防治有利于患者预后。
Objective To analyze the clinical characteristics of patients with portal vein thrombosis(PVT)and provide clinical basis for early prevention and treatment of PVT.Methods The general data,laboratory and imaging examination results,treatment and prognosis of 73 patients with PVT were analyzed retrospectively.Results Among 73 patients with PVT,39 patients had a history of endoscopic treatment and 10 patients had a history of splenectomy;in the Child classification of liver function,there were 33 cases of grade A,31 cases of grade B,and 9 cases of grade C;among the etiological factors,there were 67 cases of cirrhosis,5 cases of liver cancer and 1 case of pancreatitis;the mean level of D-dimer was(2.63±0.61)μg/mL,the mean main portal vein(MPV)diameter was(14.05±2.23)mm,and the mean portal vein blood flow velocity was(14.64±2.72)cm/s;thrombosis located in MPV in 48 cases;59 cases were treated with anticoagulation,4 cases with thrombolysis,5 cases with interventional therapy and 3 cases with surgical treatment.Conclusion The possible risk factors of PVT include hepatitis B and C virus cirrhosis,child B grade or above of liver fuction,previous endoscopic treatment and splenectomy history,widened MPV diameter and decreased portal vein blood flow velocity.Early prevention and treatment is beneficial to the prognosis of patients.
