摘要
目的分析宫腔镜检查在子宫内膜病变诊治中的临床诊断符合率。方法选取佛山市南海区人民医院2018年8月至2020年8月收治的120例疑似子宫内膜病变患者的临床资料展开回顾性研究,分别给予宫腔镜检查、阴道超声检查,将病理诊断结果作为本研究的金标准,比较宫腔镜、阴道超声诊断准确率、灵敏度、特异度、阳性预测值、阴性预测值以及疾病检出情况。结果120例患者经病理诊断,确诊118例。阴道超声准确诊断106例、宫腔镜准确诊断116例。阴道超声诊断准确率(88.33%)、灵敏度(88.98%)、特异度(50.00%)、阳性预测值(99.06%)、阴性预测值(7.14%)均低于宫腔镜(96.67%、96.61%、100.00%、100.00%、33.33%),差异均有统计学意义(P<0.05)。宫腔镜子宫内膜恶性病变、子宫内膜增生、子宫内膜炎、子宫黏膜下肌瘤、子宫内膜息肉检出率分别是23.33%、18.33%、28.33%、16.67%、10.00%,阴道超声组分别是20.83%、16.67%、26.67%、15.00%、9.17%,宫腔镜总检出率(96.67%)高于阴道超声(88.33%),差异均有统计学意义(P<0.05)。结论子宫内膜病变患者采纳宫腔镜检查,可提高诊断灵敏度、特异度,指导临床医师制定治疗方案,值得借鉴.
Objective To analyze the coincidence rate of clinical diagnosis of hysteroscopy in diagnosis and treatment of endometrial lesions.Methods The clinical data of 120 cases of suspected endometrial lesions admitted to Foshan Nanhai District People’s Hospital from August 2018 to August 2020 were selected and retrospectively researched,and hysteroscopy and transvaginal ultrasound were performed respectively.The pathological diagnosis results were regarded as the gold standard of this research,and the diagnostic accuracy,sensitivity,specificity,positive predictive value,negative predictive value and disease detection of hysteroscopy and transvaginal ultrasound were compared.Results 118 of 120 patients were confirmed by pathological diagnosis.106 cases were diagnosed by transvaginal ultrasound,while 116 cases were diagnosed by hysteroscopy.The diagnostic accuracy(88.33%),sensitivity(88.98%),specificity(50.00%),positive predictive value(99.06%)and negative predictive value(7.14%)of transvaginal ultrasound were all lower than(96.67%、96.61%、100.00%、100.00%、33.33%)of hysteroscopy respectively,with statistically significant differences(all P<0.05).The detection rates of malignant endometrial lesions,endometrial hyperplasia,endometritis,submucous myoma and endometrial polyps under hysteroscopy were 23.33%,18.33%,28.33%,16.67%and 10.00%respectively.The vaginal ultrasound group was 20.83%,16.67%,26.67%,15.00%and 9.17%,respectively.The total detection rate of hysteroscopy was 96.67%,higher than 88.33%of transvaginal ultrasound,P<0.05.Conclusion Hysteroscopy is adopted in patients with endometrial lesions,which can improve the sensitivity and specificity of diagnosis,and guide clinicians to make treatment plans,which is worthy of reference.
